妊娠中晚期上尿路结石合并肾积水临床观察

摘　　要：目的探讨妊娠中晚期尿路结石合并肾积水的诊断和治疗。方法回顾性分析2005年1月至2015年10月确诊的5例妊娠中晚期上尿路结石合并肾积水患者，经保守治疗无效后采用局麻，在膀胱镜下放置输尿管双J管引流，观察疗效。结果5例患者肾积水症状均缓解。置管过程中及术后未发生产科并发症；3例孕妇顺利足月剖宫产，1例经阴道顺产，婴儿均健康，1例因胎儿畸形行引产术；2例结石自行排出，3例产后行体外冲击波碎石术治疗。结论对于保守治疗无效的妊娠中晚期肾结石合并肾积水患者，置入榆尿管双J管支架是一种比较安全有效的方法。Objective To evaluate the diagnosis and obstetric treatment of urolithiasis with hydronephrosis during the middle and late pregnancy. Methods To retrospectively analyze 5 cases of late pregnancy with urolithiasis and hydronephrosis from January 2005 to October 2015.To place double-j-stents for ureteral drainage under cystoscope after ineffective conserVative therapy and to observe the effect. Results hydronephrosis symptoms of 5 cases were alleviated.In the process of catheterizing obstetric complications have not yet been found; 3 cases had cesarean sections, 1 case had natural birth, babies were healthy. 1 ease had labor induction because of fetal abnormalities; 2 cases discharge the stones by themselves , 3 cases were cured by ESWL after delivery. Conclusions Placing double-J-stents is a safe effective methodin treating urolithiasis with hydronephrosis during the middle and late pregnancy when conservative treatment is invalid.
