腹腔镜下子宫全切术后患者碘伏阴道残端冲洗在预防术后感染中的效果分析

摘　　要：目的探讨腹腔镜下子宫全切术后，使用稀释碘伏液冲洗阴道残端对术后感染的预防作用。方法选取2012年1月至2016年1月接受腹腔镜下全子宫切除术的患者162例，随机分为对照组（82例）和观察组（80例），术中取出子宫后。观察组采用0．2％稀释碘伏液冲洗阴道残端以及盆腹腔，对照组采用生理盐水冲洗。术后5d，观察组每日采用0．2％稀释碘伏液冲洗阴道残端，对照组则采用等量生理盐水。期间，观察两组患者的发热情况以及阴道残端感染率，从而分析稀释碘伏冲洗在预防术后感染中的效果。结果术后使用0．2％碘伏稀释液冲洗阴道残端的观察组患者阴道残端感染率（1例／1．25％）低于使用生理盐水的对照组患者（7例／8．54％），同时，观察组的术后发热率（2例／2．5％）也低于对照组（9例／10．98％），差异均具有统计学意义（P〈0．05）。结论术后采用稀释碘伏液冲洗阴道残端可降低子宫全切术的术后感染发生。Objective To investigate the preventative effect of infection of diluted iodophor solutions washing Vaginal stump after laparoscopic total hysterectomy. Methods 162 cases of patients who underwent total hysterectomy from January 2012 to January 2016 were selected and randomly divided into control group （ 82 cases ） and observation group （ 80 cases ） . After removal of uterine in the operation, 0.2% diluted iodophor solutions and normal saline were respectively used to wash vaginal stump and pelvic cavity in control group and observation group. 5 days after surgery, patients of observation group were daily given 0.2% diluted iodophor solutions to wash vaginal stump, and control group was given equal normal saline. The fever and vaginal stump infection were observed in these 5 days to analysis the preventative effect of infection of diluted iodophor solutions washing. Results The vaginal stump infection rate （ 1 case/1.25% ） of observation group using 0.2 diluted iodophor solutions to wash vaginal stump and pelvic cavity was lower than that （ 7 cases/8.54% ） of control group using normal saline. The incidence of postoperative fever （ 2 cases/2.5% ） of observed group was also lower than that （ 9 cases/10.98% ） of control group. Conclusions Using diluted iodophor solutions to wash vaginal stump can effectively prevent the infection after laparoscopic total hysterectomy.
