从太阴病论治乙型肝炎后肝硬化代偿期患者肠道微生态的临床研究

摘　　要：目的:观察理中汤治疗乙型肝炎后肝硬化代偿期患者肠道微生态的临床疗效。方法:将100例乙型肝炎后肝硬化患者随机分为治疗组52例和48对照组例,治疗组给予理中汤联合恩替卡韦分散片治疗,对照组仅予恩替卡韦分散片治疗,疗程为3个月,观察治疗前后患者肝功能、肠道菌群变化、症状积分及血清内毒、HBV-DNA转阴、HBe-Ag转阴,并探讨其可能作用机制。结果:经过理中汤治疗3个月后,治疗组患者肠道菌群变化、症状积分及血清内毒素均比对照组患者有所改善(P<0.05),但在肝功能的改善及HBV-DNA转阴、HBe-Ag转阴方面没有明显区别。结论:理中汤可显著改善太阴病脾阳虚证乙型肝炎后肝硬化患者胃肠道症状及内毒素水平,其可能通过改善肝硬化患者的肠道微生态发挥作用。Objective ： To observe the clinical efficacy of Lizhong Decoction in the treatment of hepatitis B liver cirrhosis with Taiyin spleen- Yang deficiency syndrome. Methods：A total of 100 subjects were randomized into two groups：treatment group in which 52 cases were treated with entecavir and Lizhong Decoction and control group in which 48 cases were treated with entecavir only, with a course of 3 months. Before and after treatment,liver function, the changes of intestinal flora, symptom score and serum endotoxin and HBV - DNA negative and negative HBe - Ag wee observed and to explore its possible mechanism of action. Re- sults ：After treatment of 3 months with Lizhong Decoction,the treatment group＇s intestinal flora changes, symptom score and serum endotoxin were significantly improved compared with those of the control group （ P 〈 0. 05 ）. But the liver function and HBV - DNA negative conversion ratio and HBe - Ag negative conversion ratio had no obvious difference. Conclusion ： Lizhong Decoction not only can significantly improve the Taiyin spleen - Yang deficiency syndrome of hepatitis B liver cirrhosis with gastrointestinal symptoms and the levels of endotoxin and it possibly improves the intestinal micro ecological function.
