腹腔镜下手术对卵巢肿瘤蒂扭转的治疗效果分析

摘　　要：目的分析腹腔镜下手术对卵巢肿瘤蒂扭转的治疗效果。方法选择卵巢肿瘤蒂扭转患者120例,按照随机抽签的方式将其分为观察组和对照组,观察组行腹腔镜下手术治疗,对照组行经腹保守性手术治疗,术后比较两组患者手术时间、术中出血量、术后肛门排气时间、住院时间以及术后并发症发生率,分析两组治疗的疗效,并对两组患者进行随访,了解卵巢功能情况。结果观察组的手术时间(52.6±12.3)min、术中出血量(71.4±19.1)ml、术后肛门排气时间(22.8±3.7)h、住院时间(5.4±0.8)d,均明显低于对照组(P<0.05);术后观察组的总有效率为93.3%,对照组的总有效率80.0%,两组比较差异具有统计学意义(P<0.05);观察组患者出现卵巢功能下降、卵巢供血异常、卵泡发育异常的发生率均显著小于对照组(P<0.05);两组患者术后并发症的发生率无统计学差异(P>0.05)。结论腹腔镜下手术对卵巢肿瘤蒂扭转具有显著的治疗效果,且术后恢复好,安全性高。Objective To analyze the curative effect of laparoscopic surgery in treatment of torsion of ovarian tumor. Methods A total of 120 patients with torsion of ovarian tumor were selected and randomly divided into observation group and control group. The patiertts in observation group were treated by laparoscopic surgery, while the patients in control group were treated by conservative surgery. After surgery, operation time, the amounts of blood loss during operation, postoperative anal exhausting time, hospitalization time, and incidence rates of postoperative complications in the two groups were compared, the curative effects in the two groups were analyzed, the patients in the two groups were followed up to observe ovarian function. Results The operation time, the amount of blood loss during operation, postoperative anal exhausting time, and hospitalization time in observation group were （ 52. 6± 12. 3 ） minutes, （ 71.4± 19. 1 ） ml, （ 22. 8 ±3.7 ） hours, and （5.4±0. 8 ） days, respectively, which were statistically significantly lower than those in control group （P〈0. 05 ） ; the total post0±erative effective rates in observation group and control group were 93.3% and 80. 0% , respectively, there was statistically significant difference between the two groups （ P〈 0. 05 ） ; decrease of ovarian function and abnormal ovarian blood supply occurred in observation group, the incidence rate of abnormal follicular development was statistically significantly lower than that in control group （ P〈0. 05 ） ; there was no statistically significant difference in the incidence rates of postoperative complications between the two groups （ P〉0. 05 ） . Conclusion The curative effect of laparoscopic surgery in treatment of torsion of ovarian tumor is significant, the postoperative recovery is good with high safety.
