摘 要:目的:探讨人工髋关节置换术与功能训练对老年股骨粗隆间骨折患者术后康复的效果。方法:选取2013年5月-2016年5月我院收治的老年股骨粗隆间骨折患者191例,选择2013年5月-2016年5月在我院接受治疗的股骨粗隆间骨折患者191例,年龄均>65岁,根据治疗方式不同将所选研究对象分为研究组(109例)和对照组(82例)。研究组采用人工关节置换手术治疗,对照组采用内固定手术治疗,两组同时配合功能训练。观察并比较两组患者的手术时间、术中出血量、骨折愈合时间、住院时间、Harris评分及生活质量评分。结果:研究组患者手术时间、术中出血量、骨折愈合时间及住院时间均少于对照组,差异具有统计学意义(P<0.05)。两组患者术前Harris评分比较,差异无统计学意义(P>0.05);两组患者术后Harris评分均低于治疗前,且研究组低于对照组,差异均具有统计学意义(P<0.05)。两组患者术前生活质量评分比较,差异无统计学意义(P>0.05);两组患者术后生活质量评分均高于术前,且研究组高于对照组,差异均具有统计学意义(P<0.05)。研究组优良率(94.12%)显著高于对照组(80.00%),差异具有统计学意义(P<0.05)。两组并发症比较,差异无统计学意义(P>0.05)。结论:人工关节置换术配合功能训练能够改善老年股骨粗隆间骨折患者髋关节功能,提高患者术后恢复及生活质量,值得临床推广应用。Objective: To investigate the effect of artificial hip replacement and functional training on postoperative rehabilitation of elderly patients with intertrochanteric fracture. Methods: 191 elderly patients with intertrochanteric fracture were randomly divided into study group and control group from May 2013 to May 2016 in our hospital, who were randomly divided into study group and control group, cases were randomly divided into two groups. The study group was treated with artificial joint replacement surgery, the control group was treated with internal fixation, and the two groups were combined with functional training. The operation time, blood loss, fracture healing time, hospital stay, Harris score and quality of life of the two groups were observed and compared. Results: The operation time, blood loss, fracture healing time and length of stay in the study group were less than those in the control group, the difference was statistically significant (P〈0.05). Before treatment, Harris score of two groups was compared statistically significant (P〉0. 05); After treatment, Harris scores of the two groups were lower than before treatment, and study group lower than control group, the difference is statistically significant (P〈0.05). Before operation, there was no statistically significant difference about the quality of life scores in the two groups (P 〉0.05); After operation, the quality of life scores in the two group were higher than before, and the study group was higher than that of the control group with statistically significant differences (P〈0.05). Conclusion: Artificial joint replacement combined with functional training can improve the hip function in elderly patients with intertrochanteric fracture, improve postoperative recovery and quality of life, it is worthy of clinical application.
