摘 要:目的分析脑脊液外引流相关颅内感染的危险因素。方法回顾性分析我科2007-2015年收治并行脑室或腰大池引流患者的临床资料。结果 311例患者26例出现感染,感染率为8.36%。单因素分析结果显示:GCS评分≤8分、脑室外引流(特别是双侧外引流)、外引流留置时间>5 d、脑脊液漏、脑脊液漏持续时间≥1 h或>5 h、合并糖尿病、引流期间ICU住院与颅内感染有关(P<0.05,P<0.01);多因素Logistic回归分析结果显示:脑脊液漏及其持续时间、合并糖尿病是发生颅内感染的独立危险因素(P<0.05)。结论合并糖尿病、脑脊液漏及持续时间是脑脊液外引流相关颅内感染的高危因素,临床应采取相应措施降低感染发生率。Objective To analyze the related risk factors of intracranial infections after cerebrospinal fluid (CSF) drainage. Methods Clinical data of 311 patients undergoing external ventricular drainage or lumber subarachnoid drainage in our department from January 2007 to December 2015 were collected and analyzed retrospectively. Results Intracranial infection occurred in 26 out of 311 cases with an incidence of 8.36%. The univariate analysis showed that Glasgow coma scale (GCS) score ≤8, external ventricle drainage (especially bilateral drainage), CSF leakage, duration of the leakage (≥ 1 h, or even 〉5 h), diabetics, ICU hospitalization during drainage were associated with intracranial infection (P〈0.05,P〈0.01). Multiple logistic regression analysis demonstrated that diabetics, CSF leakage and duration of leakage were independent risk factors of intracranial infection after CSF drainage (P〈0.05). Conclusion Complication of diabetics, CSF drainage and duration of leakage are high-risk factors for intracranial infection in the patients undergoing CSF drainage. Relevant preventive measures should be taken to decrease the incidence of infection.
