摘 要:目的:分析妊娠期糖尿病(GDM)人群血清中脂代谢与胰岛素水平变化的相关性。方法:选取2014年1月—2015年1月收治GDM患者102例作为研究组,选取同期健康孕妇100例作为对照组,两组孕妇在孕周28~32周进行葡萄糖耐量试验,采用全自动生化分析仪检测患者空腹血糖(FBG)、空腹胰岛素(FINS)、三酰甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)等,并计算出体质量指数(BMI)、胰岛素抵抗指数(HOMA-IR),同时采取酶联免疫吸附试验测定患者血清磷酸酪氨酸衔接蛋白(APPL1)、血清脂肪细胞型脂肪酸结合蛋白(AFABP)水平。结果:研究组BMI、TG、TC、HDL-C、LDL-C血脂指标与对照组比较,差异显著(P〈0.05);研究组血清APPL1、AFABP水平比较与对照组比较,差异显著(P〈0.05);研究组FBG、FINS、HOMA-IR与对照组比较,差异显著(P〈0.05);经Pearson相关分析,研究组HOMA-IR与BMI、TG、TC、LDL-C、APPL1、AFABP成呈正相关(P〈0.05)。结论:GDM人群发生脂代谢紊乱,其胰岛素抵抗与高血糖水平有关,因此临床治疗过程中应合理膳食,改善胰岛素不足,有效控制血糖水平,对改善母婴结局有重要和意义。Objective: To analyze the correlation between serum lipid metabolism and insulin level in patients with gestational diabetes mellitus. Methods: 102 patients with GDM in our hospital from January 2014 to January 2015 were selected as the study group; and 100 cases of healthy pregnant women were selected as the control group at the same time. Both groups took the glucose tolerance test during 28-32 weeks. The fasting blood glucose (FBG), fasting insulin (FINS), triglyceride, total cholesterol (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C) of patients were detected by the full automatic biochemical analyzer. The body mass index (BMI) and insulin resistance index (HOMA-IR) were calculated; at the same time, the level of serum tyrosine binding protein (APPL1) and the serum adipocyte fatty acid binding protein (AFABP) of patients were determined by the enzyme linked immunosorbent assay. Results: The difference of BMI, TG, TC, HDL-C, LDL-C lipid indexes between groups was significant (P 〈 0.05); The difference of serum APPL1, AFABP levels between groups was significant (P〈 0.05); In addition, the difference of FBG, FINS, HOMA-IR between groups was significant (P 〈 0.05); through Pearson correlation analysis, there was positive correlation between the HOMA-IR and the BMI, TG, TC, LDL-C, APPL1, AFABP in the study group (P 〈 0.05). Conclusion: Insulin resistance in GDM patients was related to high blood sugar levels.Therefore, diet should be reasonable; the insulin deficiency should be improved; and the levels of blood glucose should be effectively controlled, which were important and significant to improve maternal and infant outcomes.
