软伤方熏蒸治疗腰椎骨关节炎(风寒湿痹证)的临床疗效观察

摘　　要：目的:观察软伤方熏蒸治疗风寒湿痹证腰椎骨关节炎的安全性及临床疗效,为推广应用提供临床依据。方法:分别采用中药熏蒸+腰椎牵引、安慰剂熏蒸+腰椎牵引治疗风寒湿痹证腰椎骨关节炎患者60例,对比两组患者治疗前后VAS疼痛评分、改良后JOA评分、中医证候评分及治疗结束3个月后患者VAS疼痛评分。结果:1治疗结束后临床控制率分别为33.3%、10.0%,显效率分别为86.7%、43.3%,差异有统计学意义(P<0.05);2治疗结束后治疗组VAS、中医体征积分低于对照组,差异有统计学意义(P<0.05);治疗组改良JOA总积分、腰部自觉症状评分及日常生活动作评分高于对照组,差异有显著统计学意义(P<0.01);两组腰椎活动功能评分、体征评分比较,差异无统计学意义(P>0.05);3两组患者治疗结束后3个月复查复发率比较差异有统计学意义(P<0.05);4有不良事件发生,但无需特殊处理可继续治疗。结论:软伤方熏蒸治疗腰椎骨关节炎(风寒湿痹证)具有良好的临床疗效性;安全性评定为Ⅱ级,比较安全。Objective： To observe the safety and clinical efficacy of Ruanshang Formula Chinese medicine fumigation in the treatment of lumbar osteoarthritis （wind, cold and dampness syndrome）, and to provide clinical basis for the clinical popularization and application. Method： 60 patients with lumbar osteoarthritis （wind, cold and dampness syndrome） were divided into the treatment group who were treated with traditional Chinese medicine fumigation and traction treatment and the control group who were treated with placebo fumigation and traction treatment. The data of JOA, VAS and TCM symptom were collected before and 3 months after treatment. Results： （1）The Clinical control rate of the treatment group and the control group was 33.3% and 10.0% respectively, the significant efficiency rates was 86.7% and 43.3% respectively, and the difference.was statistically significant （P 〈 0.05）; （2）After treatment, the VAS score and TCM symptom score of the treatment group were lower than that of the control group, and the difference was statistically significant （P 〈 0.05）; JOA score, waist self-conscious symptom score and the score of daily activities of the treatment group were higher than that of the control group and the difference was statistically significant （P 〈 0.01）; the lumbar activity function score and clinical sign score of the two groups were similar, and the difference was not statistically significant （P 〉 0.05）; （3） 3 months after the treatment, the difference of the recurrence rate of two groups of patients was statistically significant （P 〈 0.05）; （4）There were adverse events, but there was no need for special continuous treatment. Conclusion： Ruanshang Formula Chinese medicine fumigation had good clinical effect in the treatment of lumbar osteoarthritis （wind, cold and dampness syndrome） and the safety was grade II.
