中药外用联合癌性疼痛三阶梯治疗在癌痛中的疗效观察

摘　　要：目的:观察中药外用联合规范化癌痛三阶梯止痛治疗重度癌痛的疗效。方法:将84例确诊的晚期重度癌痛患者随机分为试验组、对照组两组,试验组采用中药外用联合规范化癌痛三阶梯止痛;对照组单纯给予规范化癌痛三阶梯止痛,观察4周比较疗效。结果:中医外治联合规范化癌痛三阶梯治疗止痛效果更佳,试验组NRS评分低于单纯西药组,统计学分析有显著性差异;试验组患者情绪、睡眠、生活兴趣、日常生活能力改善优于对照组;试验组在改善焦虑抑郁状态方面明显优于对照组(P<0.005)。结论:中药外用和规范化癌痛三阶梯治疗有互补作用,能提高止痛疗效,在改善症状、提高生活质量方面有独特及显著的疗效。Objective： To observe the effect of external application of TCM combined with three step analgesic ladder in treating cancer pain. Method： 84 patients with severe advanced cancer pain were divided into the experimental group and the control group. The experimental group received external application of TCM combined with three step analgesic ladder while the control group received only three step analgesic ladder treatment. The clinical effect were observed after four weeks. Results： The external application of TCM combined with three step analgesic ladder had better effects in relieving cancer pain, NRS score of the experimental group was lower than that of the control group, the difference between two groups was of statistical significance. The improvement of the emotion, sleep, interest in life and ability in daily life of the experimental group was better than that of the control group, the improvement of the anxious despondent condition of the experimental group was significantly better than that of the control group （P〈0.005）. Conclusion： The external application of TCM complemented three step analgesic ladder in relieving cancer pain, the combination could improve the clinical effect, it had distinctive and remarkable clinical effect in enhancing the quality of life and relieving symptoms.
