摘  要:目的:检测支气管哮喘(简称哮喘)患者血清 IL-17、IL-4、干扰素-γ(IFN-γ)和IL-33的水平,探讨其与哮喘的相关性。方法选择北京市房山区良乡医院2014年5月至2015年11月收治的93例哮喘患者作为观察组,根据患者的临床表现和分期分为急性发作期组52例和缓解期组41例,选择同期健康体检人群50名作为对照组,比较各组血清 IL-17、IL-4、IFN-γ、IL-33、免疫球蛋白 E (IgE)水平变化及患者的肺功能,分析细胞因子与患者肺功能的相关性。结果急性发作期、缓解期和对照组患者的血清 IL-17、IL-4、IFN-γ、IL-33及 IgE 水平逐渐降低,差异具有统计学意义(P 〈0.05)。急性发作期组患者的 PEF、FEV1及 FEV1/FVC 均明显低于缓解期组(P 〈0.05)。哮喘患者血清 IL-17、IL-4、IFN-γ和 IL-33水平与 PEF 和 FEV1均呈负相关(P 〈0.05);而与患者 IgE值呈正相关(P 〈0.05)。结论哮喘是多种细胞因子参与的免疫炎症反应,不同细胞因子的水平升高会加重患者的肺功能障碍,相关细胞因子的监测对判断哮喘的发生、发展具有重要意义。Objective To detect the relationship between the levels of interleukin-1 7 (IL-1 7), interleukin 4 (IL-4),interferon gamma (IFN-γ) and interleukin-33 (IL-33) in serum in patients with bronchial asthma.Methods 93 patients with bronchial asthma were divided into acute phase group (52 cases) and remission phase group(41 cases),and 50 healthy cases were included as the control group.The levels of IL-1 7,IL-4,IFN-γ,IL-33 and immunoglobulin E (IgE) in serum were compared,and the relationship between cytokines and patients′ pulmonary function was analyzed.Results The levels of IL-1 7,IL-4,IFN-γ,IL-33 and IgE in acute phase group were significantly higher than those in remission phase group and control group,and also each level in remission phase group was higher than that in control group with statistical significance(P 〈0.05).PEF,FEV1 and FEV1/FVC in acute phase group were significantly lower than that in the remission phase group(P 〈0.05).Negative correlation between IL-1 7,IL-4,IFN-γ,IL-33 and FEV1 ,PEF were found in asthma patients (P 〈 0.05),however,positive correlation between IL-1 7,IL-4,IFN-γ,IL-33 and IgE were observed(P 〈0.05).Conclusions Bronchial asthma is immune inflammatory response involved in a variety of cytokines,and the elevated levels of different cytokines can aggravate the lung dysfunction,so monitoring related cytokines is of great significance to determine the occurrence and the development of asthma.
