摘  要:目的探讨乙酰半胱氨酸对老年慢性阻塞性肺疾病患者免疫功能及血清IL-8、IL-10水平的影响。方法 64例老年慢性阻塞性肺疾病急性加重期患者随机分为两组,对照组患者予常规治疗,观察组患者在此基础上联合应用乙酰半胱氨酸,比较两组患者治疗前后T淋巴细胞亚群及血清IL-8、IL-10水平的变化。结果治疗后,两组患者血清IL-8水平降低、IL-10升高,且观察组IL-8低于对照组,IL-10高于对照组,差异均有统计学意义(P<0.05);观察组患者血清CD3+、CD4+及CD4+/CD8+比值较治疗前升高,且高于对照组,差异有统计学意义(P<0.05);观察组患者FEV1/FVC、FEV1/预计值较治疗前明显提高,且高于对照组水平,差异有统计学意义(P<0.05)。结论乙酰半胱氨酸治疗老年慢性阻塞性肺疾病急性加重期患者可有效抑制炎症反应,改善患者免疫功能及肺功能。Objective To understand the effect of acetylcysteine on the immune function and serum IL-8 and IL-10 in elderly patients with chronic obstructive pulmonary disease( COPD). Method 64 elderly patients with AECOPD were randomly divided into a control group and an experimental group. The control group were treated with conventional therapy while the experimental group were treated with acetylcysteine in addition to that given to the control group. The changes in T lymphocyte subsets and serum IL-8 and IL-10 levels of the two groups before and after treatment were then compared.Findings After treatment,the serum IL-8 level of both groups became lower than before,with that of the experimental group lower than that of the control group. The level of IL-10 of both groups became significantly higher than before,with that of the experimental group higher than that of the control group. The differences were statistically significant( P〈0. 05). The CD3~+、CD^4+and CD4~+/CD8~+in the experimental group became significantly higher than before treatment and were higher than those of the control group. The differences were statistically significant( P〈0. 05). The FEV1/FVC,FEV1% pre in the experimental group became significantly higher than before treatment and were higher than those of the control group. The differences were statistically significant( P〈0. 05). Conclusion Acetylcysteine as a medicine can effectively inhibit the inflammatory response and improve the immune function and lung function of elderly patients suffering COPD.
