七氟醚复合丙泊酚麻醉用于宫腔镜手术疗效及安全性评价

摘　　要：目的探讨七氟醚复合丙泊酚在宫腔镜手术麻醉中的应用价值。方法选取医院2014年6月至2015年5月行宫腔镜手术的患者80例,随机分为观察组与对照组,各40例。观察组患者给予七氟醚复合丙泊酚麻醉,对照组患者给予瑞芬太尼复合丙泊酚麻醉,比较两组患者的手术时间、麻醉起效时间、苏醒时间、离室时间,收缩压(SBP)、舒张压(DBP)、心率(HR)、平均动脉压(MAP),以及术后不良反应发生情况。结果两组手术时间、麻醉起效时间比较,差异无统计学意义(P>0.05);观察组患者的苏醒时间为(3.05±1.14)min,离室时间为(6.05±1.16)min,分别明显短于对照组患者的(4.45±1.05)min和(7.81±1.07)min(P<0.05);两组患者麻醉前与手术结束时的SBP,DBP,HR,MAP比较,差异均无统计学意义(P>0.05);对照组患者手术开始时及手术开始10 min的SBP,DBP,HR,MAP均明显低于麻醉前(P<0.05);观察组手术开始时及手术开始10 min的SBP,DBP,HR,MAP均明显高于对照组(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论七氟醚复合丙泊酚用于宫腔镜手术麻醉效果好,患者苏醒迅速,血流动力学稳定,无呼吸抑制现象,不良反应少,具有较好的临床应用价值。Objective To explore the application value of sevoflurane combined with propofol in anesthesia of hysteroscopic operation.Methods 80 patients who underwent hysteroscopic operation in the hospital from June 2014 and May 2015 were randomized into the observation group and the control group,40 cases in each group.The observation group received sevoflurane combined with propofol anesthesia,and the control group received remifentanil combined with propofol anesthesia.The operation time,onset time of anesthesia,recovery time,off- room time,systolic blood pressure（SBP）,diastolic blood pressure（DBP）,heart rate（HR）,mean arterial pressure（MAP） and postoperative adverse reactions were compared between the two groups.Results There were no significant differences in operation time and onset time of anesthesia between the two groups（P〈0.05）;The recovery time and off-room time of the observation group[（3.05 ±1.14） min,（6.05±1.16） min]were shorter than[（4.45±1.05） min and（7.81±1.07） min]of the control group（P〈0.05）.Before anesthesia and at the end of operation,there were no significant differences in SBP,DBP,HR and MAP between the two groups（P〈0.05）;At the beginning of the operation and after 10 min of operation,SBP DBP,HR and MAP of the control group were all lower than those before anesthesia（P〈0.05）,and those of the observation group were higher than those of the control group（P〈0.05）.There was no significant difference in the incidence of adverse reactions between the two groups（P〈0.05）.Conclusion The anesthetic effect of sevoflurane combined with propofol in hysteroscopic operation is good.Patients can recover quickly.It also can ensure hemodynamic stability,without respiratory depression,and adverse reactions are few.It is of high clinical value.
