潜类别模型在医生、护士职业紧张分类中的应用

摘　　要：目的:通过使用潜类别模型分析医生和护士群体的不同职业紧张的主要类型,为更有效的干预医护人员职业紧张提供指导。方法:使用简易职业紧张调查问卷,在赣州市某三甲医院随机调查81名青年医生和66名青年护士,然后使用Mplus统计软件对数据进行潜类别模型聚类分析。结果:医生组可以探索性分为两种职业紧张类型,分别命名为"工作总体不满意所致职业紧张"型和"无有效缓解释放工作压力所致职业紧张"型。而护士组可以探索性分为三种职业紧张类型,可分别命名为"社会支持应对职业紧张"型、"自我调节应对职业紧张"型和"高危职业紧张"型。医生职业紧张表现类型主要与性别有关,护士职业紧张表现类型受年龄因素的影响。结论:应用潜类别模型可以有效地对医护群体职业紧张者的不同表现进行分类,进而开展具有针对性的干预措施,可以降低医护人员群体的职业紧张程度,最终更融洽地处理医患关系和提高医疗服务质量。Objective： Using Latent Class Analysis method for the occupational stress responses from doctors and nurses,and expecting to find different latent classes,and to provide advices for more effective intervention in medical staff occupational stress. Methods： A random survey was made with a simple and easy WHO occupational stress questionaire on 81 young doctors and 66 young nurses from a 3A-grade hospital in Ganzhou city,and then researcher build different latent class models for doctors and nurses respectively using Mplus software. Results： This research showed that there were two latent classes for occupational stress responses in doctor population,respectively named ＂ overall unsatisfaction ＂ occupational stress group and ＂ without effective release ＂ occupational stress group,and three latent classes in nurse population,respectively named ＂ social support to relieve＂ occupational stress group, ＂ self adjusting to relieve＂ occupational stress group and ＂ high risk＂ occupational stress group. The occupational stress latent classes of doctors was associated with gender,but age was a important factor for nurses. Conclusion： Application of latent class model can effectively to classify occupationnal stress responses from medical staff,and then to carry out targeted intervention,to reduce occupationnal stress,finally to get harmonious doctor-patient relationship and improve the quality of medical services.
