老年冠心病患者合并抑郁特点分析

摘　　要：目的:研究老年冠心病(CHD)患者合并抑郁的情况及其临床特点。方法:选择69例老年CHD患者(CHD组)和70例老年非CHD患者(非CHD组)进行汉密尔顿抑郁量表(HAMD)的评估,对两组HAMD总分及各因子分进行比较分析。结果:CHD组抑郁发生率显著高于非CHD组(36.23%比12.86%),P<0.01。与非CHD组比较,CHD组焦虑/躯体化[(1.69±0.68)分比(4.01±1.03)分]、认知障碍[(1.18±0.37)分比(1.89±0.78)分]、日夜变化[(1.17±0.23)分比(1.89±0.71)分]、睡眠障碍[(1.28±0.52)分比(2.69±0.71)分]、绝望感[(1.02±0.41)分比(3.71±0.49)分]因子分和HAMD总分[(8.49±2.31)分比(16.47±4.91)分]均显著升高,P均<0.01。结论:抑郁症状在老年冠心病患者中发生率较高,在临床工作中对老年冠心病患者应重视抑郁症状,尽早干预治疗。Objective：To study complicated depression status in aged patients with coronary heart disease（CHD）and its clinical characteristics.Methods：A total of 69 aged CHD patients（CHD group）and 70 aged non-CHD patients（non-CHD group）received assessment of Hamilton rating scale for depression（HAMD）,then factors scores and total score of HAMD were compared and analyzed between two groups.Results：Incidence rate of depression in CHD group was significantly higher than that of non-CHD group（36.23% vs.12.86%）,P〈0.01.Compared with nonCHD group,there were significant rise in factor scores of anxiety/somatization[（1.69±0.68）scores vs.（4.01±1.03）scores],cognitive disorders[（1.18±0.37）scores vs.（1.89±0.78）scores],diurnal variation[（1.17±0.23）scores vs.（1.89±0.71）scores],sleep disorders [（1.28±0.52）scores vs.（2.69±0.71）scores],hopelessness[（1.02±0.41）scores vs.（3.71±0.49）scores]and total score[（8.49±2.31）scores vs.（16.47±4.91）scores]in CHD group,P〈0.01 all.Conclusion：Incidence rate of depression symptoms is high in aged patients with coronary heart disease,physicians should pay attention to it,so as to treat it at the early stage.
