摘  要:目的:观察完全呼吸法呼吸训练改善老年慢性阻塞性肺病(COPD)稳定期患者运动耐量的效果。方法:共纳入老年稳定期COPD患者91例,随机分为常规治疗组(接受常规治疗和护理健康宣教,43例)和干预组(在常规治疗组基础上接受完全呼吸法干预,48例)。在干预前和干预1年后评估患者的肺功能、10m最大步行速度、6min步行距离(6MWD),并在两组间进行比较。结果:与干预前比较,干预组干预后第1s用力呼气容积[FEV1,(1.18±0.11)L比(1.50±0.12)L]、用力肺活量[FVC,(1.57±0.16)L比(2.08±0.19)L]、6MWD[(350.47±18.86)m比(378.68±11.58)m]和10m最大步行速度[(42.00±3.95)m/min比(46.10±5.60)m/min]均显著增加(P均<0.01),而常规治疗组干预前后上述指标无显著改变(P均>0.05);与常规治疗组比较,干预组干预后FEV_1[(1.17±0.09)L比(1.50±0.12)L]、FVC[(1.60±0.14)L比(2.08±0.19)L]、6MWD[(357.00±16.85)m比(378.68±11.58)m]和10m最大步行速度[(41.83±5.48)m/min比(46.10±5.60)m/min]均显著增加,P均<0.01。结论:完全呼吸法能有效改善COPD患者呼吸困难症状,改善肺功能,提高运动耐量,进一步改善患者生活质量。Objective:To observe effect of complete breathing method on exercise tolerance in aged patients with stable-phase chronic obstructive pulmonary disease(COPD).Methods:A total of 91 aged patients with stable-phase COPD were enrolled and randomly divided into routine treatment group(received routine treatment and nursing care education,43 cases)and intervention group(received complete breathing intervention based on routine treatment group,48 cases).Pulmonary function,10 m maximum walking speed,6min walking distance(6MWD)were assessed and compared between two groups before and one year after intervention.Results:Compared with before intervention,after intervention,there were significant rise in forced expiratory volume in one second [FEV1,(1.18±0.11)L vs.(1.50±0.12)L],forced vital capacity [FVC,(1.57±0.16)L vs.(2.08±0.19)L],6MWD[(350.47±18.86)m vs.(378.68±11.58)m]and 10 m maximum walking speed[(42.00±3.95)m/min vs.(46.10±5.60)m/min]in intervention group(P〈0.01all),but there were no significant changes in above indexes in routine treatment group(P〈0.05all);compared with routine treatment group after intervention,there were significant rise in FEV1 [(1.17±0.09)L vs.(1.50±0.12)L],FVC [(1.60±0.14)L vs.(2.08±0.19)L],6MWD[(357.00±16.85)m vs.(378.68±11.58)m]and 10 m maximum walking speed[(41.83±5.48)m/min vs.(46.10±5.60)m/min]in intervention group,P〈0.01 all.Conclusion:Complete breathing method can effectively improve dyspnea,lung function and exercise tolerance,then further improve quality of life in COPD patients.
