麻疹患儿外周血辅助性T细胞17及其效应因子白介素-17的表达及临床意义

摘　　要：目的:探讨麻疹患儿外周血辅助性T细胞17(Th17)及其效应因子白介素(IL)-17的表达及临床意义。方法:采用流式细胞仪(FCM)检测20例普通麻疹患儿(无并发症)、15例麻疹并发细菌性肺炎患儿、20例健康对照组儿童外周血Th17细胞的百分率;酶联免疫吸附法(ELISA)检测血清IL-17、IL-6的水平;并分析了Th17百分率与血清C反应蛋白(CRP)、降钙素原(PCT)的水平的关系。结果:麻疹并发细菌性肺炎组Th17细胞百分率均值为(4.15±0.76)%、高于普通麻疹患儿(1.71±0.63)%及对照组(1.59±0.58)%(P<0.05)。IL-17、IL-6、CRP、PCT在麻疹合并细菌性肺炎组分别为:(78.04±12.67)、(35.23±10.36)pg/mL、(3.97±0.56)ng/mL和(37.05±9.27)mg/L;在普通麻疹组的均值分别为:(50.21±10.55)、(13.18±6.93)pg/mL、(0.10±0.02)ng/mL和(0.76±0.11)mg/L;在健康对照组均值分别为:(45.95±9.33)、(11.07±6.52)pg/mL、(0.06±0.01)ng/mL和(0.45±0.08)mg/L。麻疹并发细菌性肺炎组各指标均明显高于普通麻疹患儿及健康对照组,差异有统计学意义(P<0.05);普通麻疹患儿与对照组间的差异无统计学意义(P>0.05)。麻疹并发细菌性肺炎患儿外周血Th17比率与CRP、PCT水平呈正相关(r=0.63、0.67,P<0.05)。结论:麻疹患儿外周血Th17细胞及其效应因子IL-17表达升高可能与继发细菌感染有关。Objective:To investigate the expression and the clinical significance of T helper cell 17(Th17)and Interleukin-17(IL-17)in peripheral blood of children with measles.Methods:Flow cytometry(FCM)was applied to detect the expression of Th17 in peripheral blood of 20 cases of common measles(no complication was found in these children)and 15 cases of measles complicated by bacterial pneumonia,and 20 healthy children.Moreover,the protein levels of IL-17 and IL-6in plasma of three groups were measured using enzyme linked immunosorbent assay(ELISA).Relationship between the expression of Th17 and the levels of CRP,PCT was analyzed respectively.Results:The mean percentage of Th17 cell in measles complicated by the bacterial pneumonia group was(4.15±0.76),Which was significantly higher than that in the common measles group(1.71±0.63)and in the health control group(1.59±0.58)(P<0.05).The mean levels of IL-17,IL-6,CRP and PCT in the measles complicated by bacterial pneumonia group were(78.04±12.67)pg/mL,(35.23±10.36)pg/mL,(3.97±0.56)ng/mL and(37.05±9.27)mg/L,while those in the common measles group were(50.21±10.55)pg/mL,(13.18±6.93)pg/mL,(0.10±0.02)and(0.76±0.11) mg/L,and those in health control group were(45.95±9.33)pg/mL,(11.07±6.52)pg/mL,(0.06±0.01)and(0.45±0.08)mg/L.The levels of IL-17,IL-6,CRP,as well as PCT were all increased more remarkably in measles complicated by bacterial pneumonia group than those both in the common measles group and in the healthy control group(P <0.05);Moreover,there were no significant differences between the common measles group and the healthy control group(P>0.05).A positive correlation was found between the expression of Th17 and the levels of CRP,PCT in the measles complicated by bacterial pneumonia group(r=0.63,0.67;P<0.05).Conclusion:The higher expression of Th17 and IL-17 in peripheral blood of children with measles can be associated with secondary bacterial infection,and it can be regarded as one of the lab monitoring indexes of measles.
