ICU重症心力衰竭患者血浆脑钠肽表达特征及对预后的影响

摘　　要：目的探讨ICU重症心力衰竭患者的血浆脑钠肽(BNP)表达特征及对患者预后的影响。方法选取45例ICU重症心衰患者为研究组,另选取20例健康志愿者作为对照组,对两组研究对象进行血浆BNP检测,比较两组的血浆BNP水平,分析BNP水平与研究组患者预后的相关性。结果两组的心率、LVEDS、LVEF、LVEDD、LAD、肺动脉内径比较有显著性差异(P<0.05)。研究组患者的血浆BNP水平均显著高于对照组(P<0.05)。NYHAⅡ级者的BNP水平最低,Ⅳ级者的BNP水平最高。研究组中血浆BNP水平≤100 ng/L患者的死亡率和再次心衰入院率最低,≥500 ng/L患者的死亡率和再次心衰入院率最高。血浆BNP水平>500 ng/L与重症心衰患者死亡率呈正相关(P<0.05)。结论 ICU重症心力衰竭患者的血浆BNP表达水平较高,血浆BNP的表达水平会随着心功能的恶化而升高,血浆BNP水平的异常升高预示着患者的预后较差。Objective：To investigate the expression characteristics of plasma brain natriuretic peptide（BNP）of ICU patients with severe heart failure and its effect on prognosis. Methods：Forty-five cases of patients with severe heart failure ICU patients were used as the study group,and 20 cases of healthy volunteers as the control group. Both groups were detec-ted in plasma BNP,the plasma BNP levels of two groups were compared,and the correlation of BNP levels between progno-sis of patients was analysed. Results：There was a significant difference in the heart rate,LVEDS,LVEF,LVEDD,LAD and pulmonary artery between two groups（P 500 ng/ L and mortality in patients with severe heart failure were positively correlated（P 〈 0. 05）. Con-clusion：The plasma BNP expression levels of ICU patients with severe heart failure are higher. BNP level with the deterio-ration of cardiac function will rise,and abnormally elevated plasma BNP levels indicate a poor prognosis.
