摘 要:目的:应用受试者工作特征(ROC)曲线对比评价超声和磁共振对胎儿胼胝体的诊断价值。方法:选取侧脑室轻度扩张胎儿26例,四位受试者分别根据超声与磁共振图像对是否为胼胝体发育不全作出分级诊断,后根据诊断结果绘制四位受试者的ROC曲线,计算四位受试者的曲线下面积,对其进行统计检验。同时,分别计算超声和磁共振阅片时,两位受试者之间Kappa系数,以评估受试者之间的一致性。结果:超声检查获取两位受试者的ROC曲线下面积分别为0.859和0.869,磁共振检查获取两位受试者的曲线下面积为0.915和1,四条曲线下面积差异无显著性意义(P>0.05);超声和磁共振诊断时,两位受试者之间的Kappa系数分别为0.733和0.830。结论:磁共振较超声诊断胎儿胼胝体发育不全的准确性和一致性高,但超声对胎儿胼胝体发育不全亦有较高诊断价值,当超声不能明确诊断,磁共振可以作为胎儿胼胝体发育不全时的补充诊断手段。Objective:To assess the value of ultrasound and magnetic resonance(MR)in the diagnosis of agenesis of corpus callosum by receiver operating characteristic(ROC)analysis.Methods:Twenty-six fetuses suspected of agenesis of corpus callosum were examined by ultrasound and MR within forty-eight hours.The images of sonography were diagnosed by two sonologists separately.The images of MR were diagnosed by two radiologists separately.The diagnostic results were evaluated by ROC curve analysis with SPSS 19.0.The Kappa coefficient between two sonologists and two radiologists was calculated by SPSS 19.0.Results:Areas under the two ROC curves with sonography was 0.859 and 0.869.Areas under the two ROC curves with MR was 0.915 and 1.00.There were no significant difference of the areas under the ROC curve among the four ROC curves(P〉0.05).Conclusion:The image of MR can improve the diagnostic accuracy and consistency with agenesis of corpus callosum.However,sonography also has very high accuracy andconsistency in the diagnosis of agenesis of corpus callosum.MR can be a complementary tool to ultrasound in the diagnosis of agenesis of corpus callosum.
