移植肾动脉狭窄的PTA联合支架置入治疗的疗效分析

摘　　要：目的:分析经皮血管腔内成形术(PTA)联合支架置入治疗移植肾动脉狭窄(TRAS)的安全性及疗效。方法:收集我院2009年1月-2015年4月PTA联合支架置入治疗后的TRAS患者进行回顾性分析,主要观察终点是移植肾动脉再狭窄,次要观察终点是再次介入治疗、移植肾丢失、介入治疗后血肌酐值及血压进展。结果:在我院共查询到11例PTA联合支架置入治疗TRAS的患者,其中男性8例,女3例。PTA联合支架置入术后共有2例出现再狭窄,平均血压由术前154.9/93.0mmHg降至术后136.5/84.4mmHg,平均血肌酐水平由术前478.6μmol/L降至术后148.2μmol/L,平均肾小球滤过率由20.7ml/min升至45.8ml/min,手术成功率100%,术中未发生血栓、出血、移植肾栓塞等严重并发症。结论:PTA联合支架置入治疗TRAS是安全有效的,手术成功率高,并发症少,并对合并移植肾动脉假性动脉瘤破裂出血同样适用。Objective：To analysis the safety and efficiency of transplant renal artery stenosis treated by angioplasty and stent placement.Methods：All cases of TRAS admitted for treatment in our unit from January 2009 to December 2015 were reviewed retrospectively.The primary end point was no transplant renal artery restenosis.Secondary end points were no reintervention,graft lost and postoperative serum creatinine level,and blood pressure evolution.Results：A total of 11（8males,3females）cases of TRAS patients in our hospital treated by PTA combined with stent insert.Postoperative,the restenosis occurred in 2patients,the average blood pressure was reduced from 155/93 mmHg to 136/84 mmHg,average serum creatinine level decreased from 482μmol/L to 148μmol/L,the average glomerular filtration rate increased from 23 ml/min to 47 ml/min,the technical success rate was 100%,and during operation no serious complications occurred such as blood colts,hemorrhage,renal embolism.Conclusion：The PTA combined with stent treatment of TRAS is safe and presents a high rate of technical success with low morbidity.For transplanted renal artery pseudoaneurysm ruptured,TRAS can also be used.
