摘 要:目的 探讨前外侧微创全髋关节置换术的效果。方法 选择我院2012年10月~2015年12月期间收治全髋关节置换术患者60例,根据随机数字表法分为两组,前外侧入路组和传统入路组。传统入路组采取传统后外侧全髋关节置换术治疗;前外侧入路组采取前外侧微创全髋关节置换术治疗。比较两组患者的切口长度、术后下蹲时间、手术平均出血量、术后6个月髋关节功能评分和手术主要并发症发生率、髋关节功能恢复优良率。结果前外侧入路组手术主要并发症发生率明显少于传统入路组,髋关节功能恢复优良率高于传统入路组(P〈0.05)。前外侧入路组患者切口长度、术后下蹲时间、手术平均出血量、术后6个月髋关节功能评分均优于传统入路组(P〈0.05)。结论 前外侧微创全髋关节置换术效果确切,手术切口小,创伤小,可减少术中出血,减少术后并发症的发生,能够有效改善髋关节功能,值得推广。Objective To study the effect of anterolateral minimally invasive total hip arthroplasty. Methods The study data were from 60 patients with total hip arthroplasty who were admitted to our hospital from October 2012 to Decem- ber 2015. According to the random number table, they were divided into two groups, namely the anterolateral approach group and the conventional approach group. The conventional approach group was treated with conventional posterolat- eral total hip arthroplasty; the anterolateral approach group was treated with anterolateral minimally invasive total hip arthroplasty. The length of the incision, postoperative squat time, average bleeding volume during surgery, 6-month postoperative hip function score, the incidence of major complications and the excellent and good rate of joint function recovery were compared batween the two groups. Results The incidence of major complications in the anterolateral ap- proach group was significantly less than that in the conventional approach group, and the excellent and good rate of joint function recovery was higher than that in the conventional approach group(P〈O.05). The incision length, postopera- tive squat time, average blood loss, and hip function score 6 months after the surgery in the anterolateral approach group were all better than those in the conventional approach group(P〈0.05). Conclusion The anterolateral minimally invasive total hip arthroplasty is effective, with small incision and less trauma. It can reduce the bleeding during opera- tion and reduce the incidence of postoperative complications, which is able to improve the function of hip joint and is worthy of promotion.
