改良Stoppa入路手术治疗双侧耻骨支骨折临床分析

摘　　要：目的探讨改良Stoppa入路手术治疗双侧耻骨支骨折患者的临床效果及应用价值。方法 选取2010年1月~2013年10月期间我院采用改良Stoppa入路手术治疗双侧耻骨支骨折16例患者为研究对象,单独改良Stoppa入路10例、联合髂窝入路4例、联合后入路2例,记录患者手术切口长度、术中出血量及术后功能评价。结果 手术切口平均长度（9.2±0.4）cm,手术平均时间为（92.4±12.5）min,术中平均出血量（600.3±10.4）m L;术后随访（12.5±2.3）个月,伤口无化脓性感染,无螺钉松动及钢板断裂等。按Majeed骨折复位标准,优良15例。结论在双侧耻骨支骨折的临床治疗上,改良Stoppa入路手术治疗效果理想,手术视野清晰,术后恢复快,并发症少,是治疗双侧耻骨支骨折的理想手术方法。Objective To discuss the clinical effect and application value of modified Stoppa approach in the treatment of bilateral pubic branch fractures. Methods 16 patients with bilateral pubic branch fractures treated with modified Stoppa approach from January 2010 to October 2013 were selected as the study subjects. 10 cases were given modified Stoppa approach alone, 4 cases were given combined iliae fossa approach, and 2 cases were given combined posterior approach. The length of operation incision, the amount of blood loss during operation, the evaluation standard of frac- ture reduction and the postoperative functional evaluation were recorded. Results The average length of incision was （9.2±0.4） cm; the average operation time was （92.4±12.5） min and the average blood loss during operation was （600.3± 10.4） mL; the patients were followed up for（12.5±2.3） months. There was no purulent infection in the wound, no loosen- ing of the screws or fractures of the plate. According to Majeed fracture reduction criteria, 15 cases were excellent and good. Conclusion The modified Stoppa approach is ideal for the clinical treatment of bilateral pubic branch fractures. Surgical vision is clear, postoperative recovery is rapid, complications are less, and it is the ideal surgical method for the treatment of bilateral pubic bone fractures.
