探讨人工全髋关节置换术(THA)在进行性髋关节发育不良(DDH)继发骨性关节炎中的治疗效果

摘　　要：目的探究对进行性髋关节发育不良(DDH)继发骨性关节炎患者采用人工全髋关节置换术(THA)治疗的临床疗效。方法对该院2011年7月—2015年7月收治的60例采用THA治疗的DDH继发骨性关节炎患者进行观察,对所有患者进行为期1年的随访,治疗前后运用Harris评分评定患者的膝关节功能,观察所有患者的治疗效果及不良反应发生情况。结果所有患者的Harris评分由术前的(43.8±5.0)分、(39.5±3.8)分、(36.2±2.9)分、(28.7±3.4)分上升到(94.5±1.9)分、(90.9±4.1)分、(85.0±3.3)分、(79.4±4.3)分;手术前后Harris评分差异有统计学意义(P<0.05)。随访期间所有患者均未出现假体松动或脱落、深静脉血栓等严重并发症,恢复情况良好,患者缩短长度明显减小。结论 THA对DDH继发骨性关节炎患者的疗效显著,可以纠正髋关节畸形,减轻患者的疼痛,提高患者的生活质量,具有较高的临床应用价值。Objective To explore the clinical efficacy of total hip arthroplasty （THA） in the treatment of osteoarthritis sec- ondary to progressive hip dysplasia （DDH）. Methods DDH secondary osteoarthritis patients in our hospital in July 2011 to July 2015 60 cases treated with THA were observed for a period of 1 years of follow-up for all patients before and after treatment with Harris score of knee joint function of patients were assessed, observe the therapeutic effect and adverse reac- tions of all patients occurred. Results All patients with Harris score from preoperative （43.8 ± 5） points, （39.5 ± 3.8） points, （36.2 ± 2.9）points, （28.7± 3.4） points up to （94.5 ± 1.9）points, （90.9±4.1）points, （85 ± 3.3）points, （79.4 ± 4.3）points; have statistically significant differences in Harris scores before and after operation （P 〈 0.05）. All patients did not appear prosthe- sis loosening or shedding, deep vein thrombosis and other serious complications, the recovery was good, patients shorten the length of the patients were significantly decreased.Conclusion THA has a significant-effect on the patients with osteoarthri- tis secondary to DDH. It can correct the deformity of hip joint, reduce the pain of patients, improve the quality of life of pa- tients, and has a high clinical value.
