经颈静脉放置临时滤器联合健侧股静脉穿刺置管治疗急性下肢深静脉血栓探讨

摘　　要：目的探讨经颈静脉放置临时滤器联合DSA引导健侧股静脉穿刺置管溶栓治疗急性下肢深静脉血栓的疗效。方法整群选取2013年6月—2016年6月期间该院收治的96例急性下肢深静脉血栓患者作为研究对象,对照组给予传统静脉溶栓治疗,观察组给予经颈静脉放置临时滤器后,健侧股静脉穿刺置管溶栓治疗。对比分析两组患者治疗前后患肢静脉通畅评分及静脉通畅率、并发症、尿激海用量以及住院天数。结果两组患者治疗前的经脉通畅评分比较差异无统计学意义(P>0.05);观察组治疗后的静脉通畅评分(3.67±1.35)分、静脉通畅率(72.03±7.65)%、尿激酶用量(253.64±56.94)万U、住院天数(12.39±4.56)d及并发症发生率[(出血(6.00%)、发热(4.00%)]等均显著优于对照组,组间差异具有统计学意义(P<0.05)。结论经颈静脉放置临时滤器联合DSA引导健侧股静脉穿刺置管溶栓治疗急性下肢深静脉血栓具有给药少、起效快、操作简便、安全性高等优点,值得在临床推广。Objective To investigate the transjugular placement of temporary filter combined with DSA guided femoral vein puncture effect of thrombolytic therapy for acute lower extremity deep venous thrombosis. Methods Group selection 96 cas- es of acute lower extremity deep venous thrombosis in our hospital during the period from June 2013 to June 2016 as the research object, the control group was given conventional intravenous thrombolytic therapy, observation group was treated with jugular vein placed temporary filter tube after thrombolytie therapy in the contralateral femoral vein puncture. Compar- ison of two groups of patients before and after treatment limb vein and venous patency patency rates, complications, and the amount of urokinase sea hospitalized for several days.Results Two groups of patients before treatment meridians patency rates was not statistically significant （P〉 0.05）; the observation group after treatment patency rates （3.67 ± 1.35） points, the patency rate （72.03 ± 7.65）%, the amount of urokinase （253.64 ± 56.94） million U, length of hospital stay （12.39 ± 4.56） d and the incidence of complications [bleeding （6.00%）, fever （4.00%）] etc was significantly better than the control group, the difference was statistically significant （P between groups 〈 0.05）. Conclusion The left jugular vein temporary filter com- bined with DSA guided femoral vein catheterization thrombolysis in treating acute Deep venous thrombosis of the lower ex- tremity has the advantages of less administration, rapid onset, easy operation and high safety. It is worth to be popularized in clinical practice.
