局麻下改良疝环充填式无张力修补术治疗腹膜沟复合疝160例

摘　　要：目的探讨局麻下改良疝环充填式无张力复合疝修补的应用。方法方便选取回顾性分析2011年1月—2016年1月,该院收治的同侧腹膜沟斜疝与直疝并发的复合疝的患者160例,按照手术方式的不同,分为观察组和对照组,每组80例,观察组为局麻下行改良疝环充填式无张力修补术,治疗组为常规的疝环充填式无张力修补术,观察手术时间、术后并发症、住院时间、住院费用等。结果 160例患者均顺利完成手术,均在手术中证实为复合疝.观察组与治疗组患者慢性疼痛及复发等术后并发症比较,观察组发生并发症共7例,治疗组发生并发症共19例,差异有统计学意义(χ2=6.613,P=0.01<0.05),两组手术时间、住院时间、住院费用等情况比较差异无统计学意义(t=1.26、0、0,P>0.05)。结论局麻下的改良疝环充填式无张力修补术是治疗同侧腹膜沟复合疝患者较为理想的术式,临床治疗效果满意。Objective To investigate the application of improved Mesh Plug tension-free repair for compound inguinal her- nia under to ponarcosis. Methods Convenient selection 160 cases of compound hernia （indirect inguinal hernia and direct inguinal hernia in the homonymy） which underwent improved Mesh Plug tension-free repair under to ponarcosis in our hos- pital from January 2011 to January 2016 were analyzed retrospectively. Patients were assigned to observation group and control group according to different procedure and 80 cases each groups. Observation group apply improved Mesh Plug ten- sion-free repair while control group apply conventional Mesh Plug tension-free repair. Then observe the operation time, postoperative complication, length of hospitalization, cost of hospitalization and so on. Results A total of 160 cases were op- erated successfully and all confirmed to be compound hernia. There are 7 cases in observation group of complications and 19 cases in control group, but the difference in chronic pain and postoperative complications reached statistical significance between observation group and control group （X^2=6.613=0.01,P〈0.05）. There were no significant differences in the operation time, length of hospitalization and the cost of hospitalization （t=1.26,0,0,P〉0.05）. Conclusion The application of improved Mesh Plug tension-free repair for compound inguinal hernia in the homonymy under to ponarcosis have a satisfactory affect and have been shown to be an appropriate procedure
