上颈部推拿结合Mckenzie康复训练治疗青年人颈性眩晕临床观察

摘　　要：目的:观察上颈部推拿结合康复训练治疗青年人颈性眩晕的临床疗效。方法:将106例符合纳入标准的颈性眩晕患者采用随机数字表法分为观察组和对照组,各53例。观察组采用推拿手法结合康复训练(McKenzie疗法),对照组仅用推拿手法治疗。对两组患者治疗前后的颈性眩晕症状与功能进行评分,比较两组患者眩晕评分的改善情况及近、远期临床疗效。结果:两组患者在治疗后评分均较治疗前改善明显,差异均具有统计学意义(P<0.01);治疗后两组评分比较,差异具有统计学意义(P<0.05),观察组评分升高较对照组明显;治疗后两组的愈显率比较,差异具有统计学意义(P<0.05),观察组的痊愈率改善明显;远期随访观察组的复发率明显低于对照组,差异具有统计学意义(P<0.01)。结论:上颈部推拿结合康复训练治疗青年人颈性眩晕取得良好的近、远期疗效,被动治疗与主动训练有机结合,相互促进,使疗效持久。Objective： To investigate therapeutic effects of rehabilitation training combined with Tuina of upper cervical region in the treatment of youngsters with cervical vertigo. Methods： 106 cases of youngsters with cervical vertigo,in accordance with the random number table,were divided into 2 groups： an observation group and a control group. Both groups were given Tuina of upper cervical region. In addition,the observation group was treated by rehabilitation training（ McKenzie technique）. All the subjects were estimated in terms of the evaluation scale for cervical vertigo before and after treatment. The clinical short-term and long-term effects（ 1 year follow-up） were obtained. Results： Before and after treatment,the evaluation scale for cervical vertigo was significantly different in each group（ P〈0. 01）,and the score of observation group was remarkably improved. There was a significant difference in the score between two groups after treatment（ P〈0. 05）. Both groups had a good therapeutic effect,and the curative rate was significantly different between two groups（ P〈0. 05）. The long-term recurrence rate of the observation group was significantly lower than that of the control group（ P〈0. 01）. Conclusion： Compared with Tuina therapy alone,rehabilitation training combined with Tuina of upper cervical region has a superior short-term and long-term effects in the treatment of youngsterswith cervical vertigo. Active training and passive treatment can be combined and can mutually promote between each other,which can realize long-lasting effects.
