脾脏外科手术治疗方法的现状与展望

摘　　要：脾脏外科学是一门与众不同的外科学,脾脏虽不是人体存活必不可少的器官,但发挥着不可替代的作用。脾脏手术主要有脾保留性手术和全脾切除术。鉴于脾脏的功能重要,为避免"无辜性脾切除"、尽可能地保留脾脏而兴起的保留脾脏胰腺远端切除术已被广泛认可。为减少全脾切除术后的并发症及解决脾组织植入存在的一些弊端,将脾一分为二进行部分脾移植是一个可行有效的方法,部分脾移植也可用于治疗血友病甲。随着手术指征的变迁,对某些脾外伤,非手术治疗或许是更好的选择。尽管脾脏外科已相对成熟,但脾脏仍是一个值得更深入探索和研究的器官,脾脏外科也正在朝着微创、术后并发症更少的方向不断发展,腹腔镜及机器人技术也有望得到更大范围的开展。Splenic surgery is a distinct discipline, and the spleen plays an irreplaceable role despite the fact that it is not an indispensable organ in human＇ s life. Spleen surgery mainly includes splenic - preserving operation and splenectomy. Due to the significant function of the spleen, the splenic - preserving distal pancreatectomy aiming to avoid＂ innocent splenectomy＂ and preserve the spleen possibly has been widely accepted. In order to decrease the complications of the splenectomy and settle some disadvantages of splenosis, dividing the spleen into two functional parts and operating the partial splenic transplantation is feasible and effective. Moreover, the partial splenic transplantation could redress hemophilia A. As operation indications change, it may be a better choice to adopt nonoperative management. Though splenic surgery is relatively advanced, the spleen is worthy of in - depth exploration and study, and splenic surgery are steping towards minimal invasion and less postoperative complications. Additionally, laparoscopic and robotic techniques are likely to spread more extensively.
