参芪仙补汤对成人急性髓系白血病化疗后骨髓抑制的改善作用

摘　　要：目的:观察参芪仙补汤对化疗后急性髓系白血病(AML)骨髓抑制的改善作用。方法:采用随机数字表法将60例AML患者分为观察组(标准DA方案+化疗后支持治疗+参芪仙补汤)和对照组(标准DA方案+化疗后支持治疗),每组30例,化疗后连续治疗30 d。观察2组治疗后诱导缓解有效率、外周血象(程度、时间)、血制品输注、毒副反应(感染、电解质、肝功能、黏膜)、生活质量评分的变化情况。结果:1)诱导有效率:观察组有效率为90.0%,对照组有效率为83.3%,2组比较无统计学差异(P>0.05);2)外周血象:2组患者治疗后白细胞计数、血红蛋白、血小板计数均较治疗前明显改善(P<0.05);且观察组3项指标改善均优于对照组(P<0.05);观察组血小板缺乏时间明显短于对照组,差异显著(P<0.01);3)血制品输注:观察组输注红细胞、血小板的次数均低于对照组(P<0.05);4)毒副反应:观察组感染、电解质、肝功能、黏膜等指标改善均明显优于对照组(P<0.05);5)生活质量评分:2组患者疗后均较疗前生活质量评分明显增高,且观察组疗后评分明显高于对照组,差异有统计学意义(P<0.05)。结论:参芪仙补汤可明显促进AML患者化疗后造血功能的恢复,减少血制品输注,减轻感染、肝脏损害、黏膜破溃及电解质紊乱等毒副反应,使患者生存质量得到进一步改善。Objectives： To observe the improvement effect of Shenqi Xianbu Tang on myelosuppression of acute myeloid leukemia patients after chemotherapy. Methods： 60 Cases with acute myeloid leukemia were divided into two groups according to random number table,each group contained 30 cases,observation group patients were treated with DA regiment,support therapy and Shenqi Xianbu Tang; control group patients were only treated with DA regiment and support therapy,two group were treated for 30 days. The induced remission rate,routine blood indexes（ level,time）,blood products for infusion,toxic and adverse reaction（ infection,electrolyte,liver function,mucosa） and life quality score were all observed and compared between the two groups. Results： 1）Induced remission rate： Total effective rate of the observation group was 90. 0% and the control group was83. 3%,but without statistically difference between the two groups（ P〈0. 05）. 2） Routine blood indexes： WBC and PLT count,Hb level were all improved obviously in the two groups compared with before treatment（ P〈0. 05）,the effects of observation group were better than those in control group（ P〈0. 05）; 3） Blood products for infusion： The times and the amount of blood transfusion of the observation group were lower than those in the control group（ P〈0. 05）. 4） Toxic and adverse reaction： The infection,electrolyte,liver function and mucosa were all better than those in control group,the differences were statistically significant（ P〈0. 05）. 5） The life quality score： the score in the two groups were raised obviously after treated,the score of observation group was higher than that of control group（ P〈0. 05）. Conclusion： Shenqi Xianbu Tang can promote the hematopoietic functionof acute myeloid leukemia patients with chemotherapy. to recover,decrease the infusion of blood products,reduce the adverse reaction such as infection,liver injury,mucosalulceration and electrolyte disorder,etc. improve the quality of life.
