摘 要:目的:分析普通X射线机升级为数字化X射线摄影(DR)后在尘肺病筛查等特殊健康检查中应用的可行性。方法:采用随机数表法抽取在职业病健康体检期间行普通X射线机高千伏摄片的136名疑似尘肺征象者进行DR摄片,将受检者的2次摄片分别统计影像的甲级片率,同时评价尘肺病小阴影分布的检出率。结果:图像质量比较,升级后DR摄片的甲级片率为93.38%,升级前高千伏摄片的甲级片率为63.38%,升级前后甲级片率比较有明显差异(x^2=27.49,P<0.01)。尘肺病小阴影分布的检出率,升级后DR摄片组小阴影2个肺区的检出率为44.85%,升级前高千伏摄片组小阴影2个肺区检出率为22.79%,升级前后检出率比较有差异(x^2=14.78,P<0.01)。结论:在进行尘肺病的筛查诊断时,升级后DR摄取的胸片图像质量及对尘肺病小阴影分布的检出率明显优于升级前的高千伏摄片,而DR系统操作简单,成像快捷,适合尘肺病的X射线诊断。Objective: To study the feasibility of digital radiography(DR) upgraded from regular x-ray photography in occupational health examination such as pneumoconiosis screening. Methods: By random numbers table method, 136 suspected pneumoconiosis people were diagnosed by high-kv radiogaphs on regular x-ray machine and we got their radiographs on DR, counted the rate of first level of two images of subjects, while evaluating detection rate of small shadow distribution of pneumoconiosis. Results: The quality of the image: the rate of first level image was 93.38% by DR and 63.38% by high-kv radiography, which showed significant differences(x^2=27.49, P〈0.01). The detection rate of small shadow distribution of pneumoconiosis: the detection rate of small shadow in 2 lung regions was 44.85% by DR and 22.79% by high-kv radiography, which showed significant differences(x^2=14.78, P〈0.01). Conclusion: When screening and diagnosing pneumoconiosis, the quality of the image on small shadow distribution by DR was clearly better than high-kv radiography. DR system was simple to use and fast to image which was suitable for x-ray diagnostic of pneumoconiosis.
