摘 要:目的通过追踪方法学联合失效模式与效应分析对非内分泌科胰岛素泵的使用和护理进行追踪监测,建立非内分泌科胰岛素泵的质量管理方法,并检验其应用效果。方法根据追踪方法学的理论基础,确定追踪路线以及考核内容,应用追踪方法学联合失效模式与效应分析对流程进行风险评估。制订具体改进措施,比较实施此质量体系前后风险优先指数(risk priority number,RPN)值、胰岛素泵护理质量指标合格率、患者满意度、年度不良事件发生次数。结果应用此体系进行非内分泌科胰岛素泵管理之后,RPN值显著下降(P<0.01),胰岛素泵护理质量指标合格率以及患者满意度显著提高(P<0.01),年度安全隐患次数由9次下降到2次。结论追踪方法学联合失效模式与效应分析的结合使用有利于非内分泌科胰岛素泵的护理管理。Objective To establish the insulin pump system of quality management in non-diabetic divisions by using tracking methodology combined with FMEA to follow up patients with insulin pump in the non-diabetic divisions. Methods According to the theoretical basis of tracking methodology,we first determined track route and the assessment content. And then,by the application of failure mode and effect analysis,we performed risk assessment and established improvement measures,and compared risk RPN values,pass rate of insulin pump nursing quality indicator,patient satisfaction ,and annual number of potential safety hazard before and after the application of quality system. Results After tracking methodology was applied in nursing quality improvement,the RPN values decreased significantly(P〈0.01) ,the insulin pump nursing quality and patient satisfaction improved significantly(P〈0.01),the number of potential safety hazard decreased from 9 times to 2 times. Conclusion Tracking methodology combined with failure mode and effect analysis is helpful to the long-term management of insulin pump nursing quality in non-diabetic divisions.
