癌症后生育忧虑量表的汉化及信效度研究

摘　　要：目的汉化癌症后生育忧虑量表（Reproductive Concerns After CancerScale，RCAC），并分析其测评年轻女性癌症患者生育忧虑的信效度。方法翻译汉化量表，形成中文版RCAC。选取380例年轻女性癌症患者，应用中文版RCAC测评其生育忧虑水平．并评价量表的信、效度。结果量表内容效度指数为0．91，各条目内容效度指数为0．80～1．00。探索性因子分析共提取6个公因子，可解释总变异的71．118％。验证性因子分析结果显示模型拟合度良好：卡方自由度比（x2／df）为1．237；比较拟合优度指数为0．976，适配度指数为0．927，增值拟合指数为0．977，非标准拟合指数为0．970．残差均方和平方根为0．035，渐进残差均方和平方根为0．035。中文版RCAC总分与患者健康问卷-9总分呈正相关（r=0．547，P〈0．001），与癌症治疗功能评价系统一普适性量表总分呈负相关（r=-0．519，P〈0．001），聚合效度良好。量表总Cronbach’s α系数为0．792，各维度Cronbach’s α系数为0．720—0．864，重测信度为0．956，各因子重测信度为0．815～0．890。结论中文版RCAC在年轻女性癌症患者中具有较好的信度和效度，可作为中国年轻女性癌症患者生育忧虑的测评工具。Objective To translate the Reproductive Concerns After Cancer scale（RCAC）into Chinese,and assess its reliability and validity in Chinese young female cancer survivors. Methods The RCAC was translated into Chinese,and 380 young female cancer survivors were recruited to assess reliability and validity of the Chinese version of RCAC. Results The Scale-Content Validity Index （S-CVI）was 0.91 ,the Item-Content Validity Index （I-CVI）ranged from 0.80 to 1.00. Exploratory factor analysis extracted six factors,which could explain 71.118% of the total variance. The confirmatory factor analysis revealed a good fit model：x2/df=1.237;Comparative Fit Index （CFI）was 0.976;Goodness of Fit Index （GFI）was 0.927;Incremental Fit Index （IFI）was 0.977;Tucker-Lewis Index （TLI）was 0.970;Root Mean-square Residual （RMR）was 0.035;Root Mean Square Error of Approximation（RMSEA） was 0.035. The score of the Chinese version of RCAC was positively correlated with the score of the PHQ-9 （r= 0.547,P〈0.001）and negatively correlated with the score of FACT-G （r=-0.519,P〈0.001）,which indicated acceptable convergent validity. The Cronbach＇s α for the total scale was 0.792,the Cronbach＇s α for six dimensions ranged from 0.720 to 0.864. The test-retest reliability was 0.956 for the overall scale,and 0.815-0.890 for the six dimensions. Conclusion The Chinese version of RCAC is reliable and valid,which can be used to evaluate the reproductive concerns of young female cancer survivors in China.
