基于PDCA改善CKD患者治疗饮食依从性的研究

摘　　要：目的 基于PDCA针对CKD患者治疗饮食执行过程中存在的问题,针对性地优化改良并付诸实施,提升CKD患者治疗饮食的依从性.方法 充分应用PDCA步骤,针对科室CKD患者治疗饮食管理中存在的原则把握不清、执行标准缺少、执行过程中缺少具体的方案、工具缺失等突出问题,制定针对性的措施,包括建立微信远程查房平台、利用软件为患者制定个性化饮食方案、为患者提供可操作性的控盐控油工具、为科室人员开展治疗饮食知识培训,了解患者对治疗饮食可操作性、依从性和对治疗饮食重要性、治疗饮食原则的掌握情况.结果 干预后在提升CKD患者肾病饮食的重要性、肾病饮食内容掌握情况和对肾病饮食的依从性方面都得到了提升,干预前后比较,差异有统计学意义（P〈0.05）.结论 针对科室CKD患者治疗饮食中存在的问题开展PD-CA,激发了全体护士寻求更加直接而具体的护理干预措施,有利于肾病饮食宣教中突出问题的解决.Objective For problems existed during diet treatment for CKD patients, targeted optimization and modification is made on the basis of PDCA and then they are put into practice to improve CKD patients＇ diet therapy compliance. Methods PDCA steps were made full use, and for prominent problems existed during diet treatment management for CKD patients such as principle confusion, lack of executive standard, lack of specific plan in imple-mentation, and tool missing, targeted measures were made, including building Wechat remote operation platform, working out individualized diet for patients by employing software, providing practical tools for salt and oil controlling, carrying out diet treatment training for department staff, mastering situation of patients＇ operability and compliance for diet therapy and the importance for diet treatment, and their knowledge for principle of diet treatment. Results CKD patients＇ knowledge for importance, content and compliance of nephropathy diet had been enhanced after intervention. And the difference had statistical significance when comparing with pre intervention （ P〈0. 05 ） . Conclusions PDCA is executed for problems existed during CKD patients＇ diet treatment, which inspires all nurses to seek more direct and specific nursing interventions, and contributes to the solution of prominent problems proposed in nephropathy diet propa-ganda and education.
