摘  要:目的探讨一氧化氮合酶(NOS)、神经肽Y(NPY)和胎盘凋亡细胞在妊娠期肝内胆汁淤积(ICP)发生发展中的作用。方法随机选取33例ICP患者为研究组,35例正常妊娠孕妇为对照组,采用免疫组织化学技术(IH)检测两组胎盘组织中NOS、NPY的表达情况;采用酶联免疫吸附法(ELISA)检测两组母体血浆NOS、NPY的表达水平;采用Tunnel染色技术和测定线粒体活性氧检测胎盘细胞的凋亡情况。结果 NOS在研究组胎盘的表达明显低于对照组,差异具有统计学意义(P﹤0.05);NPY在研究组胎盘的表达明显高于对照组,差异具有统计学意义(P﹤0.05);NOS和NPY在两组血浆中均有表达,NOS在研究组的血浆表达明显低于对照组,差异具有统计学意义(P﹤0.05),NPY在研究组的血浆表达明显高于对照组,差异具有统计学意义(P﹤0.05);研究组胎盘细胞的AI明显高于对照组,差异具有统计学意义(P﹤0.05);研究组胎盘组织线粒体的荧光强度达明显高于对照组,差异具有统计学意义(P﹤0.05)。结论 NOS、NPY的表达异常和胎盘细胞凋亡可能与ICP的发病、胎儿宫内窘迫和胎死宫内有关。Objective:To investigate the role of NOS,NPY and placental apoptosis in the development of ICP(Intrahepatic cholestasis of pregnancy). Methods:NOS and NPY were detected by immunohistochemistry in placental tissue and ELISA method in maternal plasma from 68 cases selected randomly(33 cases of ICP and 35 normal pregnant women). Apoptosis was detected by Tunnel and reactive oxygen species in placental tissues. Results:The expression of NOS in study group was significantly lower than the control group in the placenta,the difference was statistically significant(P﹤0.05);The expression of NPY in study group was significantly higher than the control group in the placenta,the difference was statistically significant(P﹤0.05);NOS and NPY were expressed in both groups,The expression of NOS in study group was significantly lower than the control group in the plasm,the difference was statistically significant(P﹤0.05);The expression of NPY in study group was significantly higher than the control group in the plasm,the difference was statistically significant(P﹤0.05).AI in study group was significantly higher than the control group in the placental tissue,the difference was statistically significant(P﹤0.05).The fluorescence intensity of the umbilical cord tissue in the study group was significantly higher than the control group in the umbilical tissue,the difference was statistically significant(P﹤0.05). Conclusions:The abnormal expression of NOS,NPY and apoptosis may be associated with the development of ICP,fetal distress and fetal death.
