影响封闭式负压吸引技术治疗手足皮肤软组织创伤及感染创面疗效的logistic分析

摘　　要：目的：探讨影响封闭式负压吸引技术治疗手足皮肤软组织创伤及感染创面疗效的相关因素。方法回顾性分析汕头市中心医院2010年1月至2015年12月收治的手足皮肤软组织创伤及感染创面168例患者临床资料，按照疗效情况分为观察组（n=29例）与对照组（n=139），观察组为临床疗效评价差的患者，对照组为临床疗效评价优、良的患者。对相关影响因素进行单因素及二元logistic分析。结果单因素分析结果表明，年龄、创面大小、病程、创面周围血管病变、糖尿病及治疗期间的HAMD评分可能是影响VSD治疗疗效的相关影响因素，（P＜0.01）；二元logistic分析模型结果表明高龄[Exp（B）=3.391，95%CI：1.027～6.416]、创面周围血管病变[Exp（B）=4.087，95%CI：1.281～9.659]、糖尿病[Exp（B）=4.641，95%CI：1.485～12.347]及治疗期间抑郁[Exp（B）=4.003，95%CI：1.380～11.429]是影响封闭式负压吸引技术治疗手足皮肤软组织创伤及感染创面疗效危险性因素（P＜0.05）。结论高龄、创面周围血管病变、糖尿病及治疗期间抑郁是影响封闭式负压吸引技术治疗手足皮肤软组织创伤及感染创面疗效危险性因素。Objective To explore the efficacy effect factor of vacuum sealing drainage treated hand,foot and skin and soft tissue trauma and wound infections.Methods Retrospectived analysis of 168 patients clinical data with hand,foot and skin and soft tissue trauma and wound infection from Shantou Central Hospital from January 2010 to December 2015.According to the efficacy of the case into the observation groups （n=29 patients） and control group （n=139）,the Observation group was clinical evaluation with poor patients.the control group was clinical evaluation of efficacy as excellent,good for patients.Related factors were analyzed by univariate and binary logistic analysis.Results Univariate analysis showed that,age,wound size,duration,wound peripheral vascular disease,diabetes and HAMD scores during treatment may be relevant factors influencing the VSD treatment efficacy （P〈0.01）;logistic analysis model showed that the elderly [Exp （B）=3.391,95% CI：1.027～6.416],wound peripheral vascular disease [Exp（B）=4.087,95% CI： 1.281～9.659],diabetes [Exp（B）=4.641,95% CI：1.485～12.347] and during the treatment of depression [Exp（B）=4.003,95%CI：1.380～11.429] affect enclosed suction treatment of hand,foot and skin and soft tissue trauma and wound infection efficacy risk factors （P〈0.05）.Conclusion Elderly,wound peripheral vascular disease,diabetes and depression during the treatment were the impact risk factors of vacuum sealing drainage treated hand,foot and skin and soft tissue trauma and wound infections.
