肺康复对慢性阻塞性肺疾病患者骨质疏松的作用

摘　　要：目的观察肺康复对慢性阻塞性肺疾病(COPD)患者骨质疏松作用。方法选取中重度COPD稳定期患者90例,随机分为康复组45例和对照组45例,两组患者均进行呼吸内科常规治疗,康复组在此基础上增加运动疗法和健康教育相结合的肺康复治疗。于治疗前、治疗6个月后分别采用6 min步行试验(6MWT)、骨密度(BMD)测定及肺功能对各组患者进行疗效评定。结果两组患者经6个月治疗后,康复组6MWT平均距离为(317.6±25.84)m,与组内治疗前及对照组治疗后比较,差异有统计学意义(P<0.05)。康复组的BMD较组内治疗前和对照组治疗后均显著增加,差异有统计学意义(P<0.05)。两组肺功能指标治疗前后组间及组内比较,差异无统计学意义(P>0.05)。结论长期运动疗法结合健康教育的肺康复不仅可以增强COPD患者的运动耐力,还可减轻骨质疏松,提高生活质量。Objective To investigate the effects of exercise therapy and health education on the patients with chronic ob- structive pulmonary disease（COPD） complicated with osteoporosis. Methods A total of 90 patients with moderate or severe but stable COPD were randomly divided into rehabilitation group （n = 45 ）and control group（ n = 45 ）. The patients in both groups accepted general respiratory medical therapy. The patients in the rehabilitation group also accepted addi- tional pulmonary rehabilitation therapy which consisted of exercise and health education while the patients in the control group only accepted the medical therapy. Subject complete the study, and measurements were made of 6 minute walk test （6MWT） , a pulmonary function test and bone mineral density test. Results After 6 months of treatment, the bone min- eral density in the rehabilitation group was significantly increased and significantly greater than that of the control group. Intra- group comparison and inter- group comparison of the average 6 -minute walk distance （6MWD） , however, re- veal statistically significant differences. There was no significant difference in the pulmonary function test. Conclusion Puhnonary rehabilitation can not only enhance exercise endurance, but also reduce osteoporosis and improve symptoms of the chronic disease, leading to better quality of life.
