摘  要:目的:研究喉罩通气下七氟醚复合瑞芬太尼麻醉在小儿尿道下裂手术中的应用效果。方法择期行尿道下裂成形术的患儿60例,年龄2~11岁,随机分为七氟醚复合瑞芬太尼喉罩通气组(A 组),七氟醚麻醉喉罩通气组(B 组)和七氟醚复合瑞芬太尼气管插管组(C 组)。观察各组插管(喉罩)前,插管(喉罩)后30 s、5 min,切皮时,手术开始后30 min、60 min 及拔管时的血压、心率及脉搏血氧分压(SPO2);手术时间,术后清醒及出室时间;术中及术后不良反应,如术中肢动反应、术后呛咳屏气、恶心呕吐、咽痛及躁动等。结果与 C 组相比,A、B 组诱导较为平稳;与 B、C 两组相比,A 组在术中维持及拔管(喉罩)前后较为平稳。B、C 组术中及术后不良反应发生率大于 A 组。结论喉罩通气下七氟醚复合瑞芬太尼麻醉,气道管理方便,有效抑制应激反应,不良反应少,可安全有效地应用于小儿尿道下裂手术。Objective To study the efficacy of laryngeal mask airway with sevoflurane combined with remifentanil anaesthesia on children with hypospadias operation. Methods Sixty children aged from 2 to 11 years old were randomly divided into laryngeal mask airway with sevoflurane combined with remifentanil anaesthesia(group A),laryngeal mask airway inhalation sevoflurane(group B),endotracheal intubation with sevoflurane combined with remifentanil anaesthesia( group C),with 20 cases in each group. Changes of circulation at each time,operation time,recovery time and the incidences of vomiting, movement,cough,move restlessly,sore throat were recorded. Results Compared with group C,the in-duction of anaesthesia in group A and B were smooth. Compared with group B and C,the continuous an-esthesia and recovery were smooth. And less incidence of untoward effect were observed during operation group A. Conclusions Laryngeal mask airway with sevoflurane combined with remifentanil anaesthesia has advantages of quicker recovery with less adverse effects,while it can also make airway management easy and suppress stress reaction effectively in children undergoing hypospadias operation.
