小青龙汤治疗肺炎临床疗效系统评价

摘　　要：目的 :系统评价小青龙汤治疗肺炎的临床疗效与安全性。方法 :通过检索CNKI、VIP、Wan Fang Data等国内中文数据库自建库至2015年3月公开发表使用小青龙汤治疗肺炎的临床随机或半随机对照试验的文献,将符合纳入标准的研究通过系统Rev Man5.3统计学软件进行方法学质量评价,用Meta分析、漏斗图等分析方法统计数据。结果:符合纳入标准的6项研究,Meta分析结果显示总有效率合并相对危险度(RR)及95%可信区间(CI值)为1.25[1.16,1.34];症状及体征积分中包含咳嗽改善、气喘缓解、肺部影像对比改善、体温下降、肺部湿罗音消失等方面比较的MD(95%CI)值分别为-1.32[-1.85,-0.79],-1.27[-1.57,-0.98],-1.53[-1.92,-1.13],-1.25[-1.71,-0.80],-2.27[-2.63,-1.91]。结论 :小青龙汤治疗肺炎与单纯西医的常规用药相比,具有较好的临床疗效,且在症状改善等方面都显示出较大优势。但由于所纳入研究文献的方法学质量偏低,发生偏倚性较高,因此降低了文献系统评价的安全可靠性,故需要更为合理的高质量临床试验方法来验证。Objective ：To systematically evaluate the clinical efficacy and safety of Xiaoqinglong decoction on pneumonia. Methods：The published clinical literature about Xiaoqinglong decoction in treating pneumonia with randomized or semi-randomized controlled trials were retrieved by CNKI,VIP and WANFANG Data from the time of building the databases to March,2015. The methodological quality of inclusive trails was assessed by RevManS.3 software,Meta-analysis,funnel plot analysis. Results：Total 6 studies were selected. Meta-analysis indicated that the overall effective rate,the risk rate（RR） with 95% confidence interval（C/） was 1.2511.16,1.34]. The symptom and sign scores included：MD value（95% CI）in the relief of cough and dyspnea,comparison with pulmonary images,temperature changes and disappearance of wheeze were -1.32 [-1.85 ,-0.79 ] ,-1.27 [-1.57 ,-0.98 ] ,-1.53 [-1.92,-1.13 ] ,-1.25 [-1.71 ,-0.80] ,-2.27 [-2.63 ,-1.91 ]. Conclusion：Xiaoqinglong decoction was compared to the routine western medicine for treating pneumonia with high curative effects on improving symptoms,but the low methodological quality and high partiality reduced the safety and reliability of the systematic review,so the more appropriate and high level methodology was needed.
