摘  要:目的分析艾滋病病毒和乙肝病毒(HIV/HBV)共感染患者外周血T淋巴细胞亚群计数、白细胞和肝功能血清生化指标的变化。方法收集17例HIV/HBV共感染患者(共感染组)、40例HIV单纯感染者(单纯感染组)及32例健康体检人员(对照组)的抗凝血及血清,用流式细胞仪检测外周血CD4+T及CD8+T淋巴细胞,全自动血细胞分析仪检测白细胞,全自动生化分析仪检测肝功能血清生化指标。结果与对照组比较,共感染组、单纯感染组CD4+、CD8+、CD4+/CD8+、CD4+/CD3+、CD8+/CD3+、CD4+CD8+、CD4+CD8+/CD3+、淋巴细胞(LYMPH)等8个项目T淋巴细胞亚群和白细胞计数比较,差异均有统计学意义(P<0.05)。丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)和γ-谷氨酰氨基转移酶(GGT)三组间比较差异均有统计学意义(P<0.05)。三组白细胞(WBC)、中性粒细胞(NEUT)、总蛋白(TP)、白蛋白(ALB)、总胆红素(TB)、直接胆红素(DB)差异均无统计学意义(P>0.05)。结论 HIV/HBV共感染患者在疾病进程中与T淋巴细胞亚群及LYMPH、AST、ALT、GGT、ALP密切相关,对评价HIV/HBV共感染患者的免疫状况、疾病进展和疗效等方面具有重要作用。Objective To analyze the changes of counts of peripheral T lymphocyte subsets, white blood cells andserological indexes of liver function of patients with hepatitis B virus and human immunodeficiency virus co- infection.Methods CD4^+T and CD8^+T lymphocytes in peripheral blood were detected by flow cytometry, white blood cells were detectedby the automatic hematology analyzer, and serological indexes of liver function were detected by automatic biochemicalanalyzer in 17 HIV/HBV cases(a co-infection group), 40 HIV infected cases(an HIV group), and 32 healthy volunteers(acontrol group). Results The 8 indexes, CD4^+, CD8^+, CD4^+/CD8^+, CD4^+/CD3^+, CD8^+/CD3^+,CD4^+CD8^+, CD4^+CD8^+/CD3^+, LYMPHin the co-infection group and HIV group had statistically significant differences compared with those in the control group(allP〈0.05). ALT, AST, ALP, r-GGT had statistically significant difference in the co-infection group compared with that in theHIV group and control group(P〈0.01). There were no significant differences among the 3 groups in WBC, NEUT, TP, ALB,TBand DB(all P〉0.05). Conclusion The disease process of HIV/HBV co-infection patients is closely related to the counts of Tlymphocyte subsets, LYMPH, AST, ALT, GGT and ALP. Therefore, they are important indexes in the evaluation of immunestatus, disease progression and prognosis of HIV/HBV co-infection.
