祛瘀化痰汤联合西药治疗慢性肾炎随机平行对照研究

摘　　要：[目的]观察祛瘀化痰汤联合西药治疗慢性肾炎疗效。[方法]使用随机平行对照方法,将60例门诊及住院患者按抽签法简单随机分为两组。对照组30例控制血压：血管紧张素转化酶抑制剂ACEI,或血管紧张素Ⅱ受体拮抗剂ARB,合并高血压选用钙通道阻滞剂CCB或β受体阻滞剂,使血压控制在135/85mm Hg以下。抗血小板聚集：潘生丁150~？200mg/d分次服用（从小量开始,逐渐加大）。治疗组30例祛瘀化痰汤（瓜蒌15g,陈皮、姜半夏各6g,茯苓10g,丹参15g,川芎6g,当归10g,黄芪15g,生山楂、莱菔子、泽泻、生地各10g）,水煎200m L,1剂/d,早晚温服;西药治疗同对照组。连续治疗30d为1疗程。观测临床症状、血清氧化低密度脂蛋白、三酰甘油、胆固醇、血肌酐、血尿素氮、24h尿蛋白定量、不良反应。连续治疗3疗程,判定疗效。[结果]治疗组显效6例,有效18例,无效6例,总有效率80.00%;对照组显效4例,有效11例,无效15例,总有效率50.00%;治疗组疗效优于对照组（P〈0.05）。血清氧化低密度脂蛋白治疗组改善优于对照组（P〈0.05）。TG、Scr两组无显著差异（P〉0.05）,TC、BUN治疗组改善优于对照组（P〈0.01,P〈0.05）。24h尿蛋白定量两组均有改善（P〈0.05）,治疗组改善优于对照组（P〈0.05）。[结论]祛瘀化痰汤联合西药治疗慢性肾炎,疗效满意,无严重不良反应,值得推广。[Objective] To observe Quyu Huatan Tang combined with western treatment of chronic nephritis. ]Method] Random parallel control method, 60 outpatients and inpatients by lottery method is simple and randomly divided into two groups. 30 patients to control blood pressure： angiotensin-converting enzyme inhibitors ACEI, angiotensin ]] receptor antagonist or ARB, calcium channel blockers and hypertension choose CCB or [3 -blockers, blood pressure control in 135/85 mmHg or less. Anti-platelet aggregation： dipyridamole 150-200 mg/d divided doses （start small, gradually increasing）. Treatment group of 30 Quyu Huatan Tang（Trichosanthes 15g, tangerine peel, ginger Pinellia the 6g, Poria 10g, Salvia 15g, Chuanxiong 6 g, angelica 10g, Astragalus 15g, hawthorn, radish seed, Alisma, habitat 10g）, water fried 200 mL, 1 Ji/d, sooner or later, warm clothes; western medicine with the control group. 30d for a course of continuous treatment. Observation of clinical symptoms, serum oxidized low- density lipoprotein, triglycerides, cholesterol, serum creatinine, blood urea nitrogen, 24 h urinary protein, adverse reactions. Continuous treatment of 3 courses, to determine efficacy. [Result]TTreatment group 6 cases, effective in 18 cases, 6 cases, the total efficiency of 80.00%; control group, 4 cases were markedly effective in 11 cases, 15 cases, the total efficiency of 50.00%; the treatment group than the control group （P〈0.05）. Serum oxidized LDL were improved in the treatment group than the control group （P〈0.05）. There was no significant difference between TG and Scr two groups （P〉0.05）. The improvement of TC and BuN in the treatment group was better than that in the control group （P〈0.01, P〈0.05）. 24h urine protein were improved in both groups （e〈0.05）, to improve the treatment group than the control group （P〈0.05）.
