犀角散联合三棱针穴位点刺放血治疗湿热瘀阻痛风随机平行对照研究

摘　　要：[目的]观察犀角散联合三棱针穴位点刺放血治疗湿热瘀阻痛风疗效。[方法]使用随机平行对照方法,将44例门诊患者按就诊顺序方法简单随机分两组。对照组22例秋水仙碱1mg,4次/d;布洛芬300mg,2次/d早晚饭后服用。治疗组22例犀角散(水牛角30g,白茅根、鸡血藤、车前子、泽泻各20g,大青叶、马齿苋、蒲公英各15g,黄连、升麻、栀子、茵陈、黄柏、黄芩各12g,青黛10g),1剂/d,水煎150mL,分3次口服;三棱针穴位点刺放血(大椎、双侧肺俞、双侧俞、双侧脾俞、双侧膈俞、双侧太白、双侧曲池、双侧足三里),连续治疗10d为1疗程。观测临床症状、血尿酸、不良反应。治疗1疗程,判定疗效。[结果]治疗组治愈13例,显效8例,无效1例,总有效率95.45%。对照组治愈5例,有效11例,无效6例,总有效率71.43%。治疗组疗效优于对照组(P<0.05)。[结论]犀角散加味配合穴位放血治疗湿热瘀阻痛风,疗效满意,无严重不良反应,值得推广。[Objective] Observation of rhinoceros horn decoction combined with acupoint bloodletting in treating damp heat and blood stasis type gout effect. [Methods] Using random parallel control method, 44 patients by the visiting sequence method is simple and were randomly divided into two groups. Control group （22 cases） treated with colchicine tablets 1 mg, 4 times/d after taking and Ibuprofen Sustained Release Capsule 300 mg, 2 times daily morning and evening meals. The treatment group 22 cases of rhinoceros horn powder （cornu bubali 30g, Rhizoma imperatae, Caulis Spatholobi, semen plantaginis, Alisol each 20g, Folium Isatidis, purslane, dandelion each 15g, coptis root, Cimicifuga rhizome, gardenia, Artemisia capillaris, Cortex Phellodendri, Radix Scutellariae 12g, Qingdai 10g）, Daily 1 agent, water decoction 150ml, 3 times a day orally combined with acupoint bloodletting therapy （Dazhui, bilateral lung acupoints, bilateral acupoints, bilateral acupoints of spleen, bilateral Geshu, bilateral Taibai, the bilateral Quchi （lil 1）, bilateral Zusanli） continuously for l0 days for a course of treatment. Observation of clinical symptoms, adverse reactions. One course of the treatments determine efficacy. [Results] The treatment group cured 13 cases, 8 cases were markedly effective, ineffective in 1 case, the efficiency of 95.45%. In the control group were cured in 5 cases, effective in 11 cases, invalid in 6 cases, the total efficiency was 71.43%. Treatment group curative effect surpasses the control group （P〈0.05）. [Conclusion] Huoxue Zhitong decoction Treatment of blood stasis, satisfactory curative effect, no serious adverse reactions, worthy of promotion.
