远端蒂皮瓣的血液循环特征及临床意义

摘　　要：目的探究远端蒂皮瓣的血液循环特征及临床意义。方法收集30例行远端蒂皮瓣术患者的临床资料,取前臂与小腿新鲜肢体标本,从动静脉血流构筑与血流动力学两方面探究血液循环特征与意义。结果将远端蒂皮瓣供血方式分为四种:体被组织血管网(丛)、肌间隙筋膜穿血管、直接皮肤血管、知名血管,均没有出现逆向血流,与生理性更贴合。结论观察静脉回流是否顺畅是决定皮瓣能否成活的前提,蒂部浅静脉能将远端静脉血导入皮瓣内,由浅到深将回流负荷加重,增加了皮瓣成活率。Objective To explore the distally based flap.Methods The clinical data of 30 cases of patients with distal pedicle flap, fresh limb specimen of forearm and leg, the characteristics and signiifcance of blood circulation in two aspects of the driven vein blood flow structure and hemodynamics.Results The distally based lfap blood supply is divided into four types： body is organized vascular network （cluster）, muscle interstitial fascia, direct skin blood vessel, well-known blood vessel, there was no reverse flow, more fit with physiological.ConclusionIt is a prerequisite for the survival of the lfap to observe whether the venous relfux is smooth, Superifcial venous blood can be introduced into the lfap, from shallow to deep will increase the return load, increase the survival rate of skin lfap.
