摘 要:目的观察持续被动运动(continuous passive motion,CPM)对兔骨关节炎(osteoarthritis,OA)软骨细胞凋亡及凋亡相关基因(p53、caspase-3、Bcl-2、Bax)表达的影响。方法 36只新西兰大白兔随机分为3组进行造模:A、对照组,B、模型组,C、治疗组,每组12只。C组兔子在术后第1d即开始进行持续被动运动(continuous passive motion,CPM)治疗,8h/d,连续8w。流式细胞术检测关节软骨细胞凋亡情况。RT-PCR和Western blot方法检测关节软骨细胞凋亡相关基因(p53、caspase-3、Bcl-2、Bax)表达情况。结果与对照组软骨细胞凋亡率(39.85±9.93)%相比,模型组软骨细胞凋亡率(39.85±9.93)%显著升高(P<0.01),凋亡相关基因p53、caspase-3、Bax表达出现显著上调,Bcl-2表达出现显著下调。给予CPM治疗后,与模型组相比,治疗组软骨细胞凋亡率显著降低(P<0.01),凋亡相关基因p53、caspase-3、Bax表达出现显著下调,p53、caspase-3、Bcl-2、Bax Bcl-2表达出现显著上调。结论 CPM可通过影响兔OA膝关节软骨细胞的凋亡情况,从而改善OA病情,其机制可能与调节凋亡相关基因(p53、caspase-3、Bcl-2、Bax)表达相关,该研究为临床上利用CPM方法治疗OA提供了一定的科学依据。Objective OTo observe the effectiveness of continuous passive motion on articular cartilage cell apoptosis and expression of apoptosis related genes(p53、caspase-3、Bcl-2、Bax).Methods 36 New Zealand white rabbits were divided into three groups including control group,model group and treatment group with 12 rabbits in each group.Treatment group were done with continous passive motion at the beginning of postoperative 1D for eight hours each day and continued for eight weeks.Detection of articular chondrocyte apoptosis was measured by Flow cytometry technique.Apoptosis related genes p53、caspase-3、Bcl-2、Bax were measured by RT-PCR and Western blot.Results Cartilage cell apoptosis rate(39.85±9.93)% in model group increased significantly(P〈0.01) compared with that in control group(39.85±9.93)%.Apoptosis related genes p53,Caspase-3,Bax expression in model group also increased significantly and Bcl-2 significantly down-regulated.In treatment group,cartilage cell apoptosis rate decreased significantly compared with model group,and genes p53,Caspase-3,Bax expression decreased.Conclusion Continuous passive motion can relieve osteoarthritis of rabbits via affecting the apoptosis of articular cartilage cell,which may be related with expression of apoptosis regulated genes(p53、caspase-3、Bcl-2、Bax).and this study can draw a scientific basis on clinical treatment of osteoarthritis using CPM technique.
