沙美特罗替卡松联合孟鲁斯特对肺炎衣原体感染并哮喘患儿肺功能及炎症因子表达影响

摘　　要：目的探讨沙美特罗替卡松联合孟鲁斯特对肺炎衣原体感染并哮喘患儿肺功能及炎症因子表达的影响。方法回顾性分析自2015年3月至2016年3月沈阳军区总医院收治的128例肺炎衣原体感染并哮喘患儿的临床资料。根据治疗方法不同,将所有患儿分为A组(n=63)和B组(n=65)。A组给予沙美特罗替卡松粉吸入剂治疗,B组联合应用沙美特罗替卡松及孟鲁斯特治疗。比较两组患儿咳嗽症状积分、嗜酸性粒细胞(EOS)百分率、肺功能等指标以及血清、痰液中白细胞介素18(IL-18)、肿瘤坏死因子α(TNF-α)、嗜酸细胞阳离子蛋白(ECP)、可溶性白细胞介素2受体(s IL-2R)、免疫球蛋白E(Ig E)等水平。结果与B组比较,A组患儿咳嗽积分、EOS百分率及皮肤点刺试验结果均明显降低,差异均有统计学意义(P<0.05);而且,两组患儿血清及痰液的FVC、FEV1/FVC、IL-18、TNF-α、ECP、s IL-2R、Ig E水平比较,差异有统计学意义(P<0.05)。结论沙美特罗替卡松联合孟鲁斯特能明显改善衣原体感染并哮喘患儿的肺功能,可有效降低炎症因子。Objective To investigate the salmeterol fluticasone propionate combined montelukast on the effects of children chlamydia pneumoniae infection and pulmonary function in asthma and inflammatory factor expression. Methods A retro- spective study was performed on 128 children with chlamydia pneumoniae infection and asthma who were received in The General Hospital of Shenyang Military Command from March 2015 to March 2016. Due to different treatments, patients were divided into Group A （n = 63 ）and Group B （n = 65 ）. Patients in Group A received sand off clenbuterol and fluticasone pro- pionate powder for inhalation, and the others in Group B were given salmeterol for propinate combined montelukast. Children cough symptoms integral,eosinophils（EOS） percentage,indexes of pulmonary function and serum, sputum of interleukin 18 （IL-18）, tumor necrosis factor（ TNF-α）, acidophil cationic protein （ECP）, soluble interleukin receptor（sIL-2R） level and immunoglobulin E were compared between the two groups. Results Compared with Group B,cough score,EOS percentage and skin prick test results were significantly increased in Group A, and the differences were statistically significant （P 〈 0. 05 ） ; FVC, FEV1/FVC, IL-18, TNF-α, ECP, sIL-2R and IgE levels were compared between the two groups, and the differ- ences were statistically significant （ P 〈 0. 05 ）. Conclusion Conclusion Salmeterol and fluticasone combined with monte- lukast for children chlamydia pneumoniae infection and asthma can effectively reduce the inflammatory factors.
