摘 要:目的应用斑点追踪成像技术评价不同年龄正常儿童左室局部心肌收缩期周向峰值应变(SC)、应变率(Sr C)特征。方法将164例正常儿童分为5组,≤3岁为幼儿组(A组)24例,>3岁且≤6岁为学龄前组(B组)32例,>6岁且≤10岁为学龄期组(C组)33例,>10岁且≤13岁为青春前期组(D组)19例,以及>13岁且<18岁为青春期组(E组)56例。分别获取各组胸骨旁左室短轴二尖瓣、乳头肌及心尖水平观连续3个心动周期二维动态图像,分析并比较各组左室各节段局部心肌SC及Sr C值。结果侧壁及下壁基底段SC值随年龄增长而升高,E组较A组均增大,差异均有统计学意义(均P<0.05);侧壁、后壁及下壁中间段SC值随年龄增长而升高,E组较A组均增大,差异均有统计学意义(均P<0.05);前间隔、前壁、侧壁及后壁心尖段SC值随年龄增长而升高,E组较A组均增大,差异均有统计学意义(均P<0.05)。前间隔、后间隔基底段及中间段、前壁基底段、中间段Sr C值大致随年龄增长呈递减趋势,E组较A组均减小,差异均有统计学意义(均P<0.05)。结论不同年龄正常儿童左室局部心肌周向收缩功能分布存在不均一性,主要体现为幼儿期与青春期之间的差异,但随年龄增长具有一定的规律,表现为SC随年龄增长而递增,Sr C随年龄增长递减。Objective To evaluate the characteristic of regional circumferential strain(SC) and strain rate(Sr C) in healthy children at different age stages by speckle tracking imaging. Methods One hundred and sixty-four healthy children were divided into 5 groups:≤ 3 years old for the infant group(group A)24 cases, 〉3 years old and ≤ 6 years old for the preschool group(group B)32 cases, 〉6 years old and ≤10 years old for the school age group(group C)33 cases, 〉10 years old and ≤ 13 years old for the pre-adolescent group(group D)19 cases, 〉13 years old and 〈18 years old adolescent group(group E)56 cases.Left ventricular short axis views at the basal,middle and apex were performed,and the dynamic two-dimensional gray images in 3heart cycles were obtained. All images were analyzed off-line in GE Echo PAC workstation and left ventricular SC,Sr C of 18 segments were acquired. Results At the basal level,SC trend to increase as well as the increasing age in the lateral and inferior wall,SC in group A was significant less than that in group E(P〈0.05).SC increased as well as the increasing age at the middle level in the lateral,posterior and inferior wall,SC in group A was significant less than that in group E(P〈0.05).The apex,SC in anterior septum,anterior,lateral and posterior wall increased with the age in group A and group E were significant difference(P〈0.05).However,Sr C decreased with increasing age in septum and anterior wall at the basal and the middle level,Sr C in group A was significant larger than that in group E(P〈0.05). Conclusion The parameters of SC and Sr C are heterogeneous in healthy children at different age stages,the differences are mainly between infant and adolescent,but it has a certain regularity,which represents Sr C decreases with increasing age,and SC increases with increasing age.
