去氧孕烯炔雌醇片联合宫血宁胶囊治疗功能失调性子宫出血临床疗效

摘　　要：目的观察去氧孕烯炔雌醇片联合宫血宁胶囊治疗功能失调性子宫出血（DUB）的临床疗效。方法选取2014年1~8月同济大学附属第一妇婴保健院DUB已婚非妊娠女性患者98例,运用随机数字表法分为对照组（40例）和观察组（58例）,症状控制后,两组均随访1年。对照组在刮宫止血术后常规服用去氧孕烯炔雌醇片治疗,观察组在对照组基础上加服宫血宁胶囊（2粒/次,每日3次,口服）治疗。比较两组患者的临床疗效、控制出血时间与完全止血时间,治疗前后黄体生成素（LH）、卵泡刺激素（FSH）和雌二醇水平,并记录两组患者治疗及随访期间的不良反应发生情况。结果观察组总有效率为96.56%（56/58）,显著高于对照组的72.50%（29/40）,差异有统计学意义（P〈0.05）。治疗后观察组和对照组LH、FSH和雌二醇水平均较治疗前显著降低[观察组：（6.1±0.9）U/L比（7.6±1.3）U/L、（6.7±0.7）U/L比（7.7±1.4）U/L、（122±12）nmol/L比（206±15）nmol/L;对照组：（6.2±0.7）U/L比（7.7±1.2）U/L、（6.6±0.7）U/L比（7.6±1.5）U/L、（125±12）nmol/L比（206±15）nmol/L]（P〈0.01）;对照组和观察组治疗后的各激素水平比较差异无统计学意义（P〉0.05）。观察组患者控制出血时间和完全止血时间显著短于对照组[（15±3）h比（29±5）h,（31±6）h比（47±11）h],差异有统计学意义（P〈0.01）。观察组不良反应发生率与对照组比较差异无统计学意义[5.17%（3/58）比7.50%（3/40）,P〉0.05]。结论去氧孕烯炔雌醇片联合宫血宁胶囊治疗DUB优于单用去氧孕烯炔雌醇片治疗,值得临床推广应用。Objective To investigate the clinical efficacy of marvelon combined with gongxuening capsules tablets in the treatment of dysfunctional uterine bleeding（ DUB）. Methods Total of 98 married nonpregnant patients with DUB in Tongji University Affiliated First Maternity and Infant Hospital from Jan. 2014 to Aug. 2014 were divided into a control and an observation group according to the random number table method,40 cases in the control group,58 cases in the observation group. Both groups were followed up for 1year. The control group was administrated marvelon per oral after curettage; on the basis of which,the observation group was given gongxuening capsules（ 2 capsules / time,3 times / day,po）. The clinical efficacy,the levels of luteinizing hormone（ LH）,follicle stimulation hormone（ FSH）,estradiol,the time of control bleeding and complete hemostasis,and adverse drug reactions of the two groups were compared. Results The effective rate of the observation group was 96. 55%（ 56 /58）,significantly higher than 72. 50%（ 29 /40） of the control group（ P 0. 05）. After treatment,the levels of LH,FSH and estradiol of the observation group and control group were significantly lower than before the treatment [observation group：（ 6. 1 ± 0. 9） U / L vs（ 7. 6 ±1. 3） U / L,（ 6. 7 ± 0. 7） U / L vs（ 7. 7 ± 1. 4） U / L,（ 122 ± 12） nmol / L vs（ 206 ± 15） nmol / L; control group：（ 6. 2 ± 0. 7） U / L vs（ 7. 7 ± 1. 2） U / L,（ 6. 6 ± 0. 7） U / L vs（ 7. 6 ± 1. 5） U / L,（ 125 ± 12） nmol / L vs（ 206 ± 15） nmol / L],and the difference were statistically significant（ P 0. 01）; there was no significant difference in the hormone levels after treatment between the two groups（ P 〈0. 05）. The time to control of the bleeding and complete stop of bleeding of the observation group were shorter than the control group[（ 15 ± 3） h vs（ 29 ± 5） h,（ 31 ± 6） h vs（ 47 ± 11） h],the differences were statistically significant（ P 〉0. 01）.
