致心律失常性右室心肌病引起复杂心律失常分析

摘　　要：致心律失常性右室心肌病(ARVC)是常染色体显性遗传的原发性心肌病,临床表现为突发猝死、室性心律失常和心力衰竭。本文报道1例致心律失常性右室心肌病致房性和室性双源宽QRS心动过速、交替下传的文氏周期、室内阻滞等复杂心律失常的特殊病例,并结合临床对复杂心律失常的发生机制和鉴别诊断加以讨论。Arrhythmogenic right ventricular cardiomyopathy （ ARVC） is considered to be a primary cardiomyopathy with autoso-mal dominant inheritance, and its clinical manifestations are sudden cardiac death, ventricular arrhythmia and heart failure. This paper reports one special case with complex arrhythmia caused by ARVC, which includes dual source tachycardia of atrium and ventricle with wide QRS complex, Wenkebach Period of alternating beats and parietal block. Mechanism of complex arrhythmia and differential diag-nosis are also discussed combined with clinical experience.
