医院第3代头孢菌素类抗菌药物用量与肠杆菌科细菌耐药率相关性研究  

Correlation between the Dosage of Third-Generation Cephalosporins and the Drug Resistance of Enterobacteriaceae Bacteria in the Hospital

摘　　要：目的探讨第3代头孢菌素类抗菌药物用量与大肠埃希菌、肺炎克雷伯菌耐药率的相关性,为临床合理、有效用药提供参考。方法采用SPSS 13.0统计软件,运用Pearson分析法对大肠埃希菌、肺炎克雷伯菌的耐药性与第3代头孢菌素类抗菌药物的用药频度(DDDs)进行相关性分析。结果 2011年至2015年,医院第3代头孢菌素类抗菌药物用药频度与大肠埃希菌、肺炎克雷伯菌耐药性的相关性研究发现,2种肠杆菌科细菌对头孢他啶、头孢噻肟、第3代头孢菌素类抗菌药物使用总量不相关,但大肠埃希菌耐药率与头孢曲松用药频度呈正相关,差异有统计学意义(R=0.973,P=0.005)。结论头孢曲松的用药频度跟大肠埃希菌的耐药率呈相关关系,其他第3代头孢菌素类抗菌药物的用量与大肠埃希菌、肺炎克雷伯菌的耐药率无相关性,细菌耐药的影响因素较多,是否跟第3代头孢菌素类抗菌药物的用量有关,还需进一步论证。Objective To analyze the correlation of the drug resistance between Escherichia coli,Klebsiella pneumonia and usage of the third—generation of cephalosporins,and to provide the basis for reasonable and effective clinical drug use.Methods SPSS 13.0 statistical analysis software was ussed,and the Pearson analysis method of e.coli,klebsiella pneumoniae,resistance and DDDs of the thirdgeneration of cephalosporin drugs was applied for correlation analysis.Results In 2011-2015,the correlation study of the DDDs of the third-generation of cephalosporins and the drug resistance of e.coli and klebsiella pneumonia found out that no significant correlation was found between the total usage of the two enterobacteriaceae bacteria and ceftazidime,cefotaxime,the third-generation cephalosporin antibiotics;but the drug resistance rate of e.coli was positively correlated with the DDDs of ceftriaxone.The difference was statistically significant（R= 0.973,P = 0.005）.Conclusion The DDDs of ceftriaxone is related to the drug resistance rate of e.coli,and the dosage of the other third-generation cephalosporins and e.coli,klebsiella pneumoniae found no correlation in resistance rate.The influencing factors of bacterial drug resistance is many,and it still needs further argument on whether it is related to the dosage of the third-generation cephalosporins.
