摘 要:目的探究羟基磷灰石联合骨髓间充质干细胞在脊柱融合治疗中的作用。方法 50只健康成年Wistar大鼠,随机分为实验组和对照组。实验组即采用羟基磷灰石联合骨髓间充质干细胞治疗脊柱融合组,对照组即采用传统的自体髂骨移植修复法。对比两组Curylo得分、手触检测动度、疗效及安全、新生骨形成率及生物力学。结果骨髓间充质干细胞可通过全骨髓贴壁的方法获得,并具有多向分化潜能。两组术后Curylo得分为4、2分的例数差异无统计学意义(P>0.05),但实验组评分为3分的例数低于对照组,差异具有统计学意义(P<0.05)。两组术后8周时在检测人员手触下,脊柱具有动度的例数百分比无显著差异(P>0.05)。两组术后植入物均无排除现象,且两组的出血率和感染率之间的差异无统计学意义(P>0.05)。实验组的新生骨形成率显著高于对照组,同时实验组的最大应力和刚度值也显著高于对照组,差异具有统计学意义(均P<0.05)。结论羟基磷灰石联合骨髓间充质干细胞运用于脊柱融合的促进骨骼形成及愈合的能力明显高于自体髂骨移植。Objective To explore the hydroxyapatite joint between bone marrow mesenchymal stem cells in the role in the treatment of spinal fusion. Methods 50 healthy adult Wistar rats,according to the table were randomly divided into experimental group and control group,experimental group using hydroxyapatite joint between bone marrow mesenchymal stem cells to treat spinal fusion group and control group using traditional autogenous iliac grafts to repair the method,two groups of Curylo score,hand touch detection mobility,curative effect and safety assessment,new bone formation rate and biomechanics. Results Bone marrow mesenchymal stem cells were obtained by using the method of whole bone marrow adherent,and multi- directional differentiation potential. Two groups of postoperative Curylo score was 4,2,no statistically significant difference in the percentage of the number of cases( P〉0. 05),but the control group score for the three points was higher than that of the experimental group,the difference was statistically significant( P〈0. 05). Two groups of postoperative 8 weeks under the testing personnel hand touch,percentage of cases of mobility in spine had no significant difference,no statistically significant difference( P〈0. 05). Postoperative implants were ruled out phenomenon,the two groups and two groups the difference between the rate of bleeding and infection rate had no statistical significance( P〉0. 05). Experimental group of new bone formation rate was significantly higher than that of the control group,at the same time,the maximum stress and stiffness of the experimental group was significantly higher than that of the control group,the difference was statistically significant( all P〈0. 05). Conclusion The ability of hydroxyapatite joint between bone marrow mesenchymal stem cells used in spinal fusion to promote healing of bone formation and significantly is higher than that of the iliac crest autograft.
