醒脑开窍中药联合软通道微创血肿引流术治疗高血压脑出血的临床研究

摘　　要：目的观察醒脑开窍中药联合软通道微创血肿引流术治疗高血压脑出血的临床疗效。方法临床收集并纳入高血压脑出血患者70例,随机分入治疗组和对照组。所有患者均行定向软通道微创血肿引流术,治疗组术后第2日开始鼻饲醒脑开窍中药汤剂(干预3周)。观察两组患者术后苏醒时间、治疗前后中医症状积分、NIHSS评分、Barthel指数评分及并发症发生率。结果治疗组术后苏醒时间早于对照组(P<0.05);治疗3周后,治疗组中医症状积分、NIHSS、Barthel指数评分改善均优于对照组(P<0.05);治疗组和对照组并发症发生率均为2.86%,差异无统计学意义(P>0.05)。结论醒脑开窍中药联合软通道微创血肿引流术治疗高血压脑出血临床疗效确切,能有效促进患者术后苏醒、改善中医症状积分、对神经功能及日常生活活动能力康复具有积极作用。Objective： To observe the clinical efficacy of soft channel minimally invasive hematoma drainage joint awake brain Chinese herbs for the treatment of hypertensive cerebral hemorrhage. Methods： 70 cases of pa- tients with hypertensive cerebral hemorrhage were selected and divided into treatment group and control group based on different treatment methods. All patients were performed directional soft channel of minimally invasive hematoma drainage. From the second day after surgery the treatment group started to receive nasogastric awake brain Chinese herbs for three weeks. Before and after the treatment,the postoperative revival time,TCM symptom score,NIHSS score,Barthel index score,and the incidence of complications in the two groups of patients with were observed. Results： The waking time of the treatment group was earlier than that of the control group （P 〈 0.05） ;three weeks after treatment, the improvement of TCM symptoms integral, NIHSS and Barthel index score of the treatment group were better than that of the control group （P〈 0.05）;the complication rate of the treatment group and the control group was 3.33% and there was no statistically significant difference （P〉 0.05）. Conclu- sion. The soft channel of minimally invasive hematoma drainage joint awake brain Chinese herbs has definite clinical effect for hypertensive cerebral hemorrhage,which can effectively help the patients regain consciousness after surgery,improve TCM symptoms integral,and facilitate postoperative rehabilitation. It manifests the advantage of combination of Chinese and western medicine treatment.
