腹腔镜下全子宫切除术并发症的影响因素及其防治策略分析

摘　　要：目的探讨腹腔镜下全子宫切除术并发症的影响因素,并分析其防治策略。方法选择2011年1月~2015年6月在我院接受腹腔镜下全子宫切除术治疗的326例患者作为研究对象,对其临床资料进行回顾性分析,观察其近期和远期并发症发生情况。结果腹腔镜下全子宫切除术术后近期并发症主要为阴道残端出血、损伤性并发症,发生率为5.21%(17/326);远期并发症主要为潮热、心血管疾病和围绝经期症状,发生率为70.38%(202/287)。结论腹腔镜下全子宫切除术是一种安全性高、并发症少的手术方式。进一步减少并发症发生的关键环节在于严格掌握手术指征,熟练腹腔镜下仪器操作技术,遵守规范的手术操作流程以及注意损伤性并发症的发生。Objective To investigate complication of total hysterectomy under laparoscopy,and to analyze its control strategy. Methods 326 cases of patients with laparoscopic hysterectomy resection surgery in our hospital from January 2011 to June 2015 were chosen as the objects,and the clinical data were retrospectively analyzed. According to patients＇ single bilateral ovarian resection,they were divided into unilateral ovarian resection group,bilateral ovarian resection group and ovarian preservation group.Short-term and long-term complications occurrences after operation of the three groups were compared. Results Short-term complications after total hysterectomy under laparoscopy was mainly as vaginal stump hemorrhage,injury complications,and the occurrence rate was 5.21% （17/326）.Long-term complications were mainly as hot flashes,cardiovascular disease and perimenopausal symptoms,and the occurrence rate was 70.38%（202/287）.Compared with unilateral ovarian resection and ovarian preservation,the proportion of patients with complications in bilateral ovarian resection vcas significantly higher than that in patients with unilateral ovarian resection.There were no significant difference between patients with unilateral ovarian resection and patients with ovarian preserv^iion. Coltlelusion Laparoscopic hysterectomy is safe and less complication.The key link to further reduce the incidence of complications is strictly grasping the indications of operation,skilling laparoscopic instrument operation technology,complying with the standard operation procedures and paying attention to the occurrence of injury complications.
