早期微量喂养与全静脉营养支持在新生儿机械通气时的应用对比

摘　　要：目的探讨两种不同喂养模式在新生儿机械通气中的应用对比。方法选取2014年3月～2015年12月我院收取的危重儿机械通气者83例，并按照随机数字表法分为对照组（n=41）和观察组（n=42），前者行全静脉营养模式，后者行早期微量喂养模式。对比两种喂养模式的应用效果。结果（1）观察组、对照组患儿机械通气需时和体质量恢复时间，腹胀、胃潴留发生率相比，差异无统计学意义（P〉0．05）；（2）观察组患儿喂养耐受34例（34／42），耐受率为81．0％，显著高于对照组的22．O％（P〈0．05）；（3）观察组患儿胃肠功能紊乱发生率和坏死性小肠结肠炎发生率分别显著低于对照组（P〈0．05）；（4）观察组患儿达全量胃肠营养时间和住院需时显著地短于对照组（P〈0．05）。结论在新生儿机械通气治疗中，实施早期微量喂养模式可以提高患儿喂养耐受性，降低不良反应发生率，缩短住院时间，效果理想。Objective To explore the application comparison of two different feeding modes in neonatal mechanical ventilation. Methods 83 cases of neonatal mechanical ventilation from March 2014 to December 2015 were collected and divided into control group（n=41） and observation group （n=42） according to random digital table method. Patients in control group were given the total parenteral nutrition model, and patients in observation group were given the early minimal feeding mode. Effects of two groups were compared. Results （1）There were no statistical significance on mechanical ventilation time, body weight recovery time, abdominal distension and gastric retention between the two groups（P〉0.05）. （2）There were 34 cases of feeding tolerance in observation group, and feeding tolerance rate was 81%, which was significantly higher than that of control group with 22%（P 〈 0.05）. （3）The incidence of gastrointesfinal dysfunction and the incidence of necrotizing in observation group were respectively significantly lower than those in the control group（P 〈 0.05）. （4）The total amount of gastrointestinal nutrition time and hospitalization time in the observation group were respectively significantly shorter than those in the control group（P 〈 0.05）. Conclusion Early minimal feeding mode can improve feeding tolerance, reduce the incidence of adverse reactions, and shorten the hospitalization time. The effect is ideal.
